Job Description Job Title: Mobile Mental Health Therapist (MMHT) Job Classification: Independent Contractor (1099) Department: Mobile Mental Health Treatment Program Program: Mobile Mental Health Treatment (MMHT) Reports To: Clinical Director, Mobile Mental Health Treatment Program; Chief Clinical Officer Work Location: Field-Based (Philadelphia, PA – Community, Nursing Facilities, and Home-Based Settings) Work Arrangement: Fully Mobile / Field-Based Compensation Range: $40–$45 per billable hour FLSA Status: Exempt (Contractor) Position Status: Contract (1099) Reason for Requisition: ☐ New Position ☒ Program Expansion ☒ Program Launch ☐ Replacement ☐ Other: ___________________ Position Summary The Mobile Mental Health Therapist (MMHT) provides individual psychotherapy and clinical behavioral health treatment services to adults who are homebound, medically fragile, or residing in nursing facilities, assisted living facilities, or other long-term care environments. Services are delivered in community-based settings for individuals unable to access traditional outpatient care. The Mobile Mental Health Therapist is responsible for conducting clinical assessments, developing treatment plans, implementing evidence-based interventions, and maintaining complete and compliant clinical documentation in accordance with Dunbar Wellness Group, CBH, OMHSAS, and regulatory standards. This position requires strong clinical judgment, independence, and the ability to provide services safely and effectively in field-based environments. Target Population The Mobile Mental Health Therapist will serve individuals who: • Are homebound or shut-in due to medical or mobility limitations, • Reside in nursing facilities, assisted living facilities, or long-term care settings, • Receive Community HealthChoices (CHC), CBH, or Medical Assistance services, • Have chronic medical conditions and co-occurring behavioral health needs Essential Duties and Responsibilities Clinical Services • Provide individual psychotherapy services in community-based settings, • Conduct clinical assessments and develop individualized treatment plans, • Implement evidence-based treatment interventions, including Cognitive Behavioral Therapy (CBT), • Monitor client progress and modify treatment plans as clinically indicated, • Complete clinical documentation in ZooBook EHR, including:, • Progress Notes (within 48 hours, no later than 7 days), • Initial Assessments, • Treatment Plans and updates, • Discharge Summaries, • Maintain documentation in compliance with regulatory, audit, and billing requirements, • Ensure clinical records support medical necessity and service delivery Care Coordination • Coordinate with nursing facility staff, physicians, case managers, and caregivers, • Communicate clinical risks and concerns to supervisors, • Participate in clinical supervision and program meetings as required Minimum Qualifications Education Master’s degree in one of the following: • Social Work, • Counseling, • Psychology, • Marriage and Family Therapy One of the following: Fully Licensed Clinician: • LCSW, • LPC, • LMFT License-Eligible Clinician: • Actively pursuing clinical licensure, • Eligible to provide clinical services under supervision Required Experience • Minimum of 1 year of experience providing psychotherapy or behavioral health treatment to adults or older adults, • Experience conducting clinical assessments and treatment planning, • Experience maintaining timely and compliant documentation in an electronic health record, • Ability to work independently in field-based settings, • Ability to manage a mobile caseload Strongly Preferred Experience • Experience working with senior adults or geriatric populations, • Experience providing services to homebound individuals or shut-ins, • Experience working in nursing facilities, assisted living facilities, or long-term care environments, • Experience providing community-based, mobile, or home-based therapy, • Experience working with medically complex individuals Required Competencies • Ability to work independently without direct on-site supervision, • Strong clinical judgment and decision-making ability, • Strong documentation and organizational skills, • Professional reliability and accountability, • Ability to manage field-based clinical responsibilities Work Environment Services are provided in: • Private residences, • Nursing facilities, • Assisted living facilities, • Long-term care facilities Compliance Requirements Contractor must comply with all Dunbar Wellness Group and regulatory requirements, including: • Completion of documentation within required timelines, • Compliance with CBH, OMHSAS, HIPAA, and Medical Assistance standards Organizational Overview Dunbar Wellness Group provides behavioral health services across Philadelphia through: • Intensive Behavioral Health Services (IBHS), • Outpatient Therapy
