Position Summary Guardian Angel Home Care is seeking compassionate and motivated Physical Therapist Assistants (PTAs) to join our growing Home Health team. The PTA works under the supervision of a licensed Physical Therapist to provide medically prescribed therapy services to patients in their homes while helping improve mobility, strength, independence, and overall quality of life. Job Responsibilities • Conduct medically prescribed therapy programs involving exercise, massage, heat, water, light, and electrical modalities, • Apply diagnostic and prognostic test results related to muscle, nerve, joint, and functional performance, • Assist patients with active and passive therapeutic exercises, • Record and report patient progress and response to treatment, • Provide therapeutic treatments and recommend equipment needs as appropriate, • Collaborate with supervising Physical Therapists and interdisciplinary care teams, • Prepare and maintain accurate clinical documentation, • Participate in staff meetings, case conferences, in-services, and training programs, • Assist with discharge planning in coordination with the therapy case manager, • Communicate with supervisors regarding patient care concerns or additional service needs, • Follow infection control standards, OSHA guidelines, and agency policies, • Maintain compliance with Medicare Conditions of Participation and all regulatory requirements, • Demonstrate professionalism, accountability, and excellent customer serviceQualificationsEducation & Experience, • Graduate of a Physical Therapist Assistant program accredited by the APTA and/or AMA, • Current PTA license in the state of employment, • Minimum one (1) year of recent clinical experience preferred, • Home Health experience preferred but not requiredSkills & Abilities, • Strong clinical skills involving neuromuscular and musculoskeletal treatments, • Excellent communication and documentation skills, • Ability to work independently in the field, • Current CPR certification required, • Valid driver’s license, reliable transportation, and current auto insurance requiredBenefits, • Competitive pay, • Flexible scheduling, • 401(k), • Supportive leadership team, • Growth and advancement opportunities
