The community-based arm of the study will provide culturally relevant nutrition and physical activity and Health Coach support for pregnant and postpartum patients to improve maternal healthy weight in pregnancy and postpartum to prevent adverse maternal infant health outcomes. The Health Coach & Program Assistant will fulfill two roles at PHS, split about 60% of time in the health coach role and about 40% of the time in the Program Assistant role (time may shift depending on the needs of the program): • The CHIME Health Coach works directly with pregnant and postpartum CHIME participants to support healthy pregnancy and postpartum engagement and goal setting.
