Essential Functions On a semi-annual basis, and after a significant change in condition, perform a comprehensive in-home assessment which reflects the member’s prior history, functional capacity, and stability of health—including chronic diseases/conditions, treatment plans, current providers and the member’s identified areas of priority concerns.
