Job Description MUST BE AVAILABLE MONDAY - FRIDAY, 5:40AM - 5:45PM. SCHEDULE DEPENDENT UPON DRIVING SHIFT. Description: Direct Service Providers (DSP) are expected to provide person-centered service and support to people with developmental disabilities to help them achieve more meaningful and productive lives in compliance with State of Florida Agency for Persons with Disabilities mandates. The DSP is responsible for the care and well-being of people served in the Adult Day Training program, as well as for transporting clients to and/or from the ADT program as scheduled/needed. Qualifications: Individuals filling this position must be at least 18 years of age, have a valid driver’s license with a clean motor vehicle report, and be able to successfully pass a Level 2 background check. In addition, the DSP must have the ability to complete all tasks in the job description, including driving participants to and from the day program. Due to the nature of care given to the people being served, this position requires that the employee be able to lift, reach, twist, stoop, bend, push, pull, walk and possibly run without restrictions. One year of related experience is preferred. Program Summary: Life Skills Development Level 3 – Adult Day Training (ADT) are training services intended to support the participation of recipients in valued routines of the community that are age and culturally appropriate. Adult day training services include the provision of activities in a non-residential setting which provides training and assistance with acquisition, retention, or improvement in self-help, socialization, and adaptive skills. ADT services may include recreational and other activities to enhance social development and development of skills in performing activities of daily living and community living. The training, activities, and routine established by the ADT provider must be meaningful to the recipient and provide an appropriate level of variation and interest. Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence and personal choice. Transportation services to and from the client’s place of residence are also provided. Communicate with family members, caregivers, Waiver Support Coordinators, health care professionals, and others in a way that promotes a positive image of New Heights and the people we have the privilege to serve. § Immediately report any emergency situations or potential safety hazards to the supervisor, as well as information obtained that may affect the health, safety and/or services for a client. § Maintain appropriate dress, good hygiene, and professional behavior that reflects positively on the agency’s mission. § Reliably work scheduled hours, realizing that work hours may change depending upon the needs of the program. § Maintain a positive driving record, sufficient to meet all applicable legal and insurance requirements. § Perform other duties as directed by supervisor. Program Specific Job Requirements ü Review available information to develop a working knowledge of each person served in regard to types of differences, medical and behavioral status, personal goals, as well as training/support methods. Use this information to provide effective, individualized interactions and supervision. ü Responsibly supervise individuals to ensure safety, comfort, and protection of individual rights. This will include feeding participants who are unable to do so independently, changing participants who are incontinent, assisting participants in using the bathroom, and other daily living routines. ü Interact with clients using appropriate voice tone, level and pitch, language, gestures, and physical movements, at all times. ü Provide services and support as identified in the client’s support plan by assisting people in achieving their personal goals and desires. Offer opportunities for interaction and choices for activities that support a meaningful day. ü Consistently follow directions given for implementing and documenting training, support, and/or behavioral plans without making changes based on personal preferences or style. ü Gather information for personal outcome measures and record on goal planning and progress form, as required. ü Create and implement daily lesson plans, which reflect individuals’ needs and goals. ü Enter weekly attendance, as assigned, into iConnect or on attendance log. ü Report and/or document routine and unusual events including completing case notes, incident reports, etc. in a concise, understandable, and legible manner. ü Observe clients for signs of illness or changes in physical, social, and/or emotional health and record per protocol. ü Complete external and internal incident reports. ü Ensure a healthy, clean, and safe environment. ü Coordinate and supervise recreational activities and small group outings in the community, including transporting participants to such outings as assigned. ü Transport clients to and/or from the program, as assigned. ü Document and maintain all necessary data including completing required trip logs. ü Clean and disinfect the vehicle upon return. ü Attend required trainings. ü Perform other duties, as required/assigned.
