Job Description CDAC Behavioral Healthcare is looking for someone to join our Substance Exposed Newborn/Neonatal Abstinence Syndrome (SEN/NAS) program into Circuit 2 (Gadsden, Leon, Liberty, Wakula, Franklin, Jefferson Counties). This program supports families who are experiencing, or at risk for, SEN/NAS. CDAC is seeking one Care Coordinator to provide care coordination services to women and families with substance use concerns in Circuit 2. The position requires clinical skills and experience with substance use and mental health disorders, parenting and family issues, and community resources. Candidate should have a working knowledge on the impact to families with substance exposed newborns (SEN) and Neonatal Abstinence Syndrome (NAS). As part of the SEN Team, the Care Coordinators will coordinate and assist in addressing all the needs of the families to include substance use, mental health, medical, financial stability, and overall wellness. The Care Coordinator will work closely with community providers and with child welfare providers to advocate and assist in meeting these families' needs. It is a field-based, relational systems support role grounded in Capacity Path’s crisis-informed and resilience-oriented framework. It is about building connections that can foster healing, endurance, and strength to help others recover. In some ways this work may feel like outside sales, becuase you will be traveling to check in on you clients, building new relationships, and becoming a trusted support for them. There is nothing to sell, but that kind of interpersonal and outreach dynamics are what will drive this phase of this project. Incorporates findings in client service and nursing care plans as directed., • Receives, documents and implements/carries out Admission, Standing/Routine and ad hoc physician orders in accordance with unit procedures. Conducts “read back” process for each verbal order (EPH and PCC permitted) in accordance with Agency policy/unit procedures. Provides and documents direct nursing care as needed in accordance with unit procedures and approved Treatment Plans/physician's orders. Documents according to JCAHO standards., • Performs ongoing nursing supervision, assessment and monitoring of patients. Follows-up on client medical complaints. Ensures Family Practice Physician/ARNP is notified of clients needing admission physicals, consults, or other services. Notifies physician of changes in client's condition. Documents shift and ad hoc nursing care progress notes in accordance with unit procedures.
