Job Description Job Description Position: Residential Float Mentor Reports to: Assistant Director of In-Home Services and On-Call Supervises: No positions Primary Hours: Set based on needs of individuals in service 40 hours per week Required Qualifications: Current valid Arizona Driver's License Able to communicate effectively with individuals in service and team members using both verbal and written skills Able to use computer proficiently Able to meet all background checks and personal history checks Physical Requirements: Must be able to lift 50 pounds to help provide support and safety for individuals supported by lifting, moving, positioning, and transporting individuals served; May be required to use Hoyer Lifts, Wheelchairs, gait belt, etc.) General Responsibilities: • Provide active direct physical assistance, supervision and behavioral support to individuals served in Agency programs, • Provide opportunities for members to acquire, use and maintain skills which enable individuals served to live as independently as possible, • Maintain an on-going knowledge of the individuals served, • Maintain the privacy and confidentiality of individuals served, • Serve as a positive role model for individuals served, • Assist in the development of written objectives and skill plans, • Carry out skill development plans, recommendations, agreements, assignments, and routines as required by the Agency, • Assist individuals served in planning and participating in social, recreational, and educational activities at home and in the community, • Actively assist individuals served with problem solving, • Assist individuals served with planning, purchasing and/or preparation of food to ensure nutritional intake within the context of the budget, • Assist with and/or manage funds for individuals served as assigned, • Assist individuals served with personal hygiene, and health & safety issues that affect them., • Obtain, administer, and maintain medications for individuals served, • Provide transportation for individuals served, • Attend, document and provide feedback for meetings and/or medical appointments for individuals served, • Purchase household and hygiene supplies as required, • Operations/Equipment, • Provide routine and on-going housekeeping and home maintenance of interior and exterior of the residence to provide a neat, clean, healthy, and safe environment, • Conduct and participate in evacuation and emergency plan drills and document the results, • Work an agreed-upon schedule in an on-call capacity, up to five days per work or 40 hours, • Maintain active availability during assigned times, responding within 15 minutes to program-related communications and providing prompt support for staffing coverage, operational issues, and emergency situations., • Requests and notifies Assistant Director of leave times in accordance with Agency procedure, • Interact and communicate with individuals served, their team members, other Agency personnel, and members of the community in an appropriate and professional manner, • Document the progress/status of individuals served and maintains files and records in accordance with Agency standards using Agency record keeping systems, • Attend required Agency and Agency-related meetings as directed, • Comply with all Agency Policies and Procedures as well as all applicable regulations and laws Note This job description in no way states or implies that these are the only duties to be performed by the employee(s) incumbent in this position.
