Job Description Position Summary: The Clinical Service Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Service Manager must utilize clinical knowledge of best practices and evidence-based treatments to ensure BHSI members receive timely access to quality, medically necessary behavioral health services. Position Summary: CTT is mandated to provide services to those individuals within the mental health system who present the greatest degree of severity of symptomatology as evidenced by their multiple physical, psychiatric and legal conditions, extensive use of services and lack of follow-through with treatment. This population requires the highest level of medical necessity as defined by the State Adult Environmental Matrix. As a member of the ACT multidisciplinary team the CTT nurse is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation and support services; and sharing shift management responsibility with other staff. As a member of an ACT team, the CTT nurse provides an array of health care support to consumers, other team members and service providers. Utilizing the Community Support Program model, the CTT nurse routinely assesses the psychiatric and physical health care needs of these persons. The incumbent develops a strengths-oriented treatment/service plan that addresses consumer health care needs through the identification and utilization of community resources. Coordinates consumers' routine medical, vision, lab and dental appointments with team members. Job Description POSITION SUMMARY: The Behavioral Health Access Care Manager is responsible for coordinating and executing all activities related to high-risk and/or increased needs from within the OP Behavioral Health Department. The Access Care Manager works across FPCSN sites and interfaces with Individuals served within OP BH who are experiencing challenging transitions in their care, and/or experience higher resource needs, and performs a range of administrative duties to optimize program outcomes. They support clinical outcomes based on planning and linkage to both internal and external resources on an individualized basis. Team members are expected to uphold the health center's mission by assuring that Individuals receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCSN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress, including poverty and racism, on all those who have contact with the organization.
