Job Description Hospice & Palliative Care Registered Nurse (RN) Case Manager Job Summary: Faith Hospice & Palliative Care, a division of Faith Health Group is seeking a Registered Nurse (RN) Case Manager who will plan, coordinate and provide hospice & palliative care in accordance with the patient's plan of care.
