Job Description The Clinical Care Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Coulter Street POSITION SUMMARY The IHD Caseworker provides casework and supervision to clients and families in the In-Home Detention component of the Home-Based Delinquent Services Program. Cases from other HBDS components (In-Home Detention) may be allocated on an as needed basis. The In-Home Detention Case Worker reports directly to the Supervising Social Worker or Director of the IHD program. The position is Fee for Service and cases are allocated on an as needed basis. Cases are not guaranteed, and the position is a contracted position, therefore, hours, assignments and cases fluctuate week to week. The actual daily hours and workdays are determined between the individual IHD case worker and the supervising social worker, in order to ensure daily coverage (7 days a week). POSITION RESPONSIBILITIES • IHD casework and monitoring., • The IHD Caseworker provides monitoring and supervision of 5 contacts per youth per week in the IHD program up to 6 cases. All contacts must be documented and submitted for review on a weekly basis. Contacts consist of visits made by the IHD Caseworker to meet with the youth face to face for a brief contact or transporting a youth on an activity with other youth where IHD Caseworkers conduct educational, life-skills counseling, recreational, cultural and social activities that may benefit the client., • The IHD Caseworker meets the objectives of providing sufficient supervision to ensure the youth's appearance in court, the client’s remaining arrest-free, the observation of curfews, and school and/ or work attendance., • When a client referral is received from the JJC, the first face-to-face contact must be made within 24 hours., • At the initial meeting with the youth and the family in their home, the IHD Caseworker describes the program requirements (curfews, school attendance, and making court dates). Job Description POSITION DESCRIPTION: PHYSICAL THERAPIST POSITION SUMMARY: The Physical Therapist (PT) is a licensed health care professional that administers Physical Therapy to adult patients requiring part-time, intermittent visits and assesses their therapeutic needs. The therapist works as a member of an interdisciplinary rehabilitation team serving as care managers with responsibilities for assessing, implementing, coordinating and evaluating patient care whenever nursing is not active with the patient. The PT supervises the physical therapy assistant (PTA) who has been approved by the Commonwealth of Pennsylvania to work in an indirect supervision setting and the home health aide if nursing is not active with the patient. RESPONSIBLE TO: Director of Professional Services or designee. I. In this role, you will support individuals living with mental health challenges in finding, securing, and maintaining meaningful employment. By providing individualized coaching, job development, and ongoing support, you will play a key role in helping participants build confidence, independence, and long-term vocational success. Key Responsibilities Administrate the employment and educational services of the Open Door Clubhouse. This would include, but is not limited to, overseeing the contractual obligations of the clubhouse to the Office of Vocational Rehabilitation. • Participate in the Unit Meetings, in which clubhouse members discuss the various Work Ordered Day (WOO) tasks to be performed in the Employment/ Educational Services Unit, and volunteer to perform those tasks according to their choice., • Participate in the WOO activities of the Employment/ Educational Services Unit, working side by side with the members and supporting them in accomplishing the WOO tasks, while preparing them in Job Readiness., • Orientation of new members to the employment and educational services offered by the clubhouse., • Ensure that members have appropriate identification for employment, and provide assistance to members in need of obtaining identification., • Conduct group activities focused on such topics as: resume writing, job searching, hygiene and appropriate dress for job interviewing, practice job applications and interviewing techniques, socialization and assertiveness skills, problem solving skills, the impact of work earnings on member benefits (SSI, SSDI), as well as special topics as needed., • Make provision for members to learn to use public transportation, as needed., • Assess and evaluate individual member's interests and skills according to the member's participation in the WOO. Assist members in making employment and/ or educational choices accordingly., • Reporting of member employment and earnings to benefit providers (i.e. SSA)., • Facilitate monthly dinner meetings for members who are employed, or considering employment, for support, socialization, and an exchange of work related information pertaining to member issues., • Perform the role of Placement Manager for Transitional Employment Placements (TEP). This includes, but is not limited to the following: job coaching and providing support to the member working the TEP, acting as liaison between the employer and the member, acting as liaison between the employer and the clubhouse, provide coverage for member absences from work, and complete all paperwork contractually required by OVR., • Prepare outcome measures relevant to member employment, for purposes of reporting., • Participate in one or more of the various clubhouse committees., • Facilitate the Clubhouse Orientation sessions as assigned., • Provide all documentation for Employment/ Educational Services Unit members, as required by funding sources. CORA is currently seeking a Program Coordinator who would be responsible for the administrative responsibilities of the YouthCOR program at St. Martin de Porres School (zip code 19132). Starting Rate: $19.50 for 3 years exp and an AA degree (salary commensurate with experience and credentials) Schedule: Hours for this position are Monday - Friday from 12:00 pm - 6:00 pm (30 hours per week) All CORA team members are expected to uphold the mission, vision and values of CORA Services which includes valuing all individuals and supporting each individual’s potential for growth. In addition, as a Program Coordinator your duties will include: • Work with the Program Manager to develop and implement high quality programming within the St. Martin de Porres Catholic School - YouthCOR Year-Round OST Program., • Assist in developing a flagship program at St. Martin de Porres, providing on-site, hands-on learning and training opportunities for new staff to be prepared for high quality OST programming., • Drive program innovation, ensuring that all lesson plans and program plans are creative, grade appropriate and meet rigorous academic standards., • Accepts and fulfils material requests from staff, ensuring all staff are equipped with materials needed for high quality programming., • Connects with parents on Class Dojo, to provide open communication between the program and families., • Observe, engage, coach, and support site-based staff to ensure consistent high quality program delivery., • Assess the effectiveness of programmatic activities and make recommendations for modifications., • Serve as a point of contact for new partnerships, while maintaining strong relationships with current partners. Ensure program scheduling is smooth and communication is strong., • Support the enrollment efforts at a large program, oversee the data system of enrolling youth into CitySpan., • Support the Manager in maintaining an active Certificate of Compliance for licensing., • Oversees daily attendance, ensuring data is entered accurately and on time., • Manages the food program, ensuring the program remains in compliance with all expectations., • Perform other duties as assigned by Director of Positive Youth Development.
