Job Description The Community Health Case Manager conducts comprehensive assessments with clients and supports them in developing individualized service plans that maximize health, wellbeing, stability, and independence. This professional position works in the development and coordination of public health education, health promotion, and community engagement activities. The role provides in-the-community case management for people living with HIV through the Ryan White program and Information & Referral support for older adults recently released from incarceration through the ASOP initiative.
