Job Description Palliative Care Advanced Practice Provider (Bi-lingual, Spanish preferred) Location: Remote, Texas Who We Are: Tuesday Health is a value-based palliative care provider group dedicated to transforming serious illness and end-of-life care. The Role: The Advanced Practice Provider (APP) will act as an experienced and innovative leader in clinical care delivery, collaborating closely with the Head of Clinical Delivery and other Tuesday Health clinicians. In partnership with the Head of Clinical Delivery, Head of Operations, market operations and provider partnership teams, as well as the product and clinical teams, the APP will help ensure the provision of high-quality, patient-centered supportive care. This work aims to enhance the quality of life for patients and their families while reducing unnecessary medical expenditures during the last one to two years of life. Role Outcomes: • Reduce ED visits and hospitalizations in the panel population at company goals, • Ensure appropriate hospice transition rate and length of stay in the panel population at company goals, • Ensure patient panel meets all contractual requirements, including but not limited to: advance care planning documentation, serious illness conversations, and post-discharge visits., • Ensure care within assigned patient panel meets all clinical quality standards set by the company, • Maintain patient and caregiver satisfaction above 90% Key responsibilities of this role may include: • Provide both virtual and in-person palliative care, as well as provide virtual supervision for nurses on the clinical team, with a focus on acute care utilization avoidance and optimal hospice transitions, • Review population level data and ensure trends are captured and interventions to respond to trending data are implemented within the clinical model, • Review patient panels with nurses and conduct chart reviews to ensure appropriate and proactive care plans are being delivered, • Lead internal and joint case rounds of all members to ensure there are no barriers to care, as well as appropriately direct case review to be certain care plan development and execution is appropriate, • Coordinate with Network Providers and Longitudinal Care Providers to best coordinate services on behalf of the members, • Support provider and clinical staff training on various topics to ensure up to date knowledge sharing across the team, • Support quality management and quality improvement initiatives as identified, • Support the performance and quality of our network partners, providing feedback, guidance, support, and corrective actions as needed, • Collaborate with our clinical, operational, financial, and technology teams to ensure seamless integration and coordination of our network services across the continuum of care, • Monitor and analyze network data and metrics, such as utilization, outcomes, satisfaction, cost-effectiveness, and growth opportunities, • Travel for in-person support, company/clinical team meetings, etc, • Willingness to be licensed in multiple states if required, • Represent our company in external forums and events related to palliative care and network management, • Participate in after-hours call schedule via phone, • Stay abreast of the latest trends, innovations, best practices, and regulations in palliative care and network management A strong candidate will demonstrate the following: • Graduate of an accredited Nurse Practitioner or Physican Assistant Program, • Five years clinical experience in palliative care, or hospice, or working with patients with serious illness.
