Job DescriptionDescription: Key Recovery provides comprehensive substance use disorder treatment and work skills training to help low-income, high-risk patients achieve sustained recovery and rebuild their lives. The Medication Coordinator assists patients throughout their time in our treatment program, including providing support during the initial admission and thoroughly preparing for discharge from Key Recovery. The medication coordinator is an integral part of our clinical team by supporting the counselors in providing a trauma informed care environment and supporting the patients in their recovery journey. The medication coordinator will help patients coordinate medical care with medical prescribers, and monitor patients self-administer their medications. While preparing for discharge, the medication coordinator will assist the discharge staff to coordinate aftercare appointments with medical providers and pharmacy upon discharge. Primary Responsibilities: • Assist with admitting new patients by participating in patient medical and medication orientation, including receiving medications, entering data into electronic health records, and updating other clinical, RSS and Case Manager staff with patient needs., • Provides monitoring of patients self-administering their medications., • Collaborate with Case Managers to ensure the patients have the most updated medication prescriptions and medications onsite., • Completes incident reports related to medication errors., • Provides training and support to the RSS staff following policies and procedures for patient self-administration medication., • Develop a medical and medication service plan when needed with each patient to determine the resources available to meet the identified needs in a coordinated and integrated manner, and advocate with community service providers to assist the patient in achieving the goals of the service plan., • Monitors patient progress by verifying patient medication compliance and responses, and communicating with clinical staff team about the patients’ progress, areas of concern, etc., • Complete medication observation and documentation, scans, and uploads patient documentation into EHR., • Communicates patient progress by participating in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to clinical staff., • Run reports and recognize errors in K Comm and follow up with training as necessary., • Develop and maintain program rules and responsibilities for the patients to encourage group compatibility and to assist the patients in learning the skills necessary to progress in their recovery., • Supports discharge staff with patient discharge instructions by being available for reviewing medication needs with clinical staff, coordinating discharge and post-discharge requirements and coordinating after-care resources., • Responsible for returning unused medications to pharmacy., • Alert pharmacy of patient discharge, • Audit patient bins and report discrepancies to supervisors, • Reconcile inventory in electronic health records, • Destroy unused and expired medications properly, as well as medications for discharged patients, • Track supplies and inventory and report needs to supervisor, • Attend all mandated meetings. Education/Experience/Licensing: • Minimum high school degree or equivalent, • Certified Nursing Assistant/Medical Assistant required. LPN preferred., • AA Degree in chemical dependency studies or other relevant studies preferred, • Must have 1-2 years of experience working in residential treatment facilities, • Experience with monitoring self-administered medications, • Experience working with patients with SUD and co-occurring disabilities., • Computer and EHR literate., • Reliable work attendance and ability to work some evening and weekend hours., • Demonstrated ability to establish and maintain variety of records, organize data, prepare reports., • Knowledge of community resources and demonstrate ability to work cooperatively and responsively with them., • Ability to demonstrate Diversity, Equity and Inclusion in all work-related activities., • Ability to advocate for inclusion, equity and belonging for all patients and colleagues., • Ability to plan, prioritize and organize your own workload and work independently., • Ability to communicate effectively both in writing and orally., • Establish and maintain effective work relationships and maintain strong boundaries with clients., • Ability to teach life skills, such as budgeting and to facilitate group meetings., • Demonstrated ability to maintain current, accurate and complete documentation in patient records., • CPR and First Aid certification required or must be able to obtain within 30 days of employment, • Must be able to pass a criminal background check to be able to work around children and vulnerable adults, successfully pass preemployment TB and drug testing Requires regular standing, walking, bending, lifting, smiling, sitting, kneeling, reaching, twisting, pulling, pushing, writing, typing, crouching, and climbing stairs. Staff in this position are required to: use both hands, hold and handle items and office equipment; speak clearly, listen carefully and occasionally lift 40 pounds including moving beds, lift light furniture and baby cribs.
