Summary of the position: The Clinical Case Manager 1 is responsible for providing home-based and community-based quality services for families involved with the Department of Child Services and/or Juvenile Probation. Duties and Responsibilities: • Ensure all case records, correspondence, and conversations regarding clients remain confidential as required by HIPAA compliance regulations., • Complete releases and GJR intake forms upon initial contact with client/placement/other parties ensuring copies are saved in the clinical record., • Provide home-based services to clients in compliance with Service Standards published by the Indiana Department of Child Services for the service that they are providing., • Communicate initial client contact or lack thereof to referral source and Compliance Coordinator with 48 hours., • Document client contacts through up-to-date case notes that are maintained in Case Management Pro (electronic database), VPN client files and Kid Traks (if applicable)., • Provide monthly Progress Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) as well as any other specific written or verbal information requested by referring agencies., • Provide Treatment Plans, Safety Plans, and other required clinical documents/resources/tools within 7-30 days of referral to referral agencies. Updated Treatment Plans are due no less than every 90 days to supervisor for review and sent to referral source by pre-arranged due dates., • If applicable, complete visitation/parenting time logs and send to referral sources within 48 hours of services rendered ensuring all required sections of log are compliance with current state content standards., • Oversee completion of program required assessments, including but not limited to, PFS-2, NCFAS, UCLA PTSD, IARCA packets upon admission, discharge, three month and six-month follow-up., • Record all billable/direct and non-billable/indirect time on a daily basis, utilizing the established database while ensuring an accurate timesheet is maintained., • Document use of and adherence to evidence-based models and treatment for all services requiring use of a model that has been approved by the state of Indiana and GJR., • Obtain and submit requests for court appearances to Compliance Coordinator monthly by deadline established., • Submit Attendance and Compliance sheets for clients on caseload to Compliance Coordinator and supervisor by monthly deadline., • Appear at court hearings, case conferences, CFTM’s or staffing as necessary or requested by referral source or supervisor., • Use strength-based approaches and develop protective factors for clients including but not limited to developing parenting skills and child development knowledge, improving social and emotional competence, accessing concrete supports, building social connections, and improving resilience. Job Description Purpose: The Youth First School Mental Health Professional works closely with the respective school environment to provide a multi-tiered system of support that aligns with best practices in public health (National Institute on Drug Abuse) utilizing universal, selective, and indicated methods of prevention/early intervention in context of the Mental Health Professional role. The Mental Health Professional will follow a neuroscience and trauma informed method, building resiliency through skills, enhancing relationships with significant adults, and fostering readiness for change among individuals and throughout the school culture. Emphasis is placed on specialized mentoring used to assist students in managing life stressors/challenges through evidence-based programs and practices. Roles & Responsibilities: • Provide Prevention/Mental Health Expertise at the school advocating for early intervention/prevention, utilizing Youth First philosophy and time allocation model of service., • Provide indicated prevention/early intervention services to identified students and their families including, but not limited to:, • Assess referred students and families for service, • Develop and implement individual student/family service, • Chart student and family progress in case, • Collect and record data for program, • Serve as a liaison between the school, family, and community, • Staff case needs through clinical, • Participate in student service team (RtI, Social Emotional Team, Student Assistance) at the school to identify student needs, to support a team approach to helping students and to avoid duplication of, • Provide Small Groups (emotion regulation, social skills, )., • Plan, coordinate, and implement evidence-based programs that are the best fit for the school environment. Summary of the position: The Community-Based Case Intern is responsible for providing home-based and community-based quality services for families involved with the Department of Child Services and/or Juvenile Probation. Duties and Responsibilities: • Ensure all case records, correspondence, and conversations regarding clients remain confidential as required by HIPAA compliance regulations., • Complete releases and GJR intake forms upon initial contact with client/placement/other parties ensuring copies are saved in the clinical record., • Provide home-based services to clients in compliance with Service Standards published by the Indiana Department of Child Services for the service that they are providing., • Communicate initial client contact or lack thereof to referral source and Compliance Coordinator with 48 hours., • Document client contacts through up-to-date case notes that are maintained in Case Management Pro (electronic database), VPN client files and Kid Traks (if applicable)., • Provide monthly Progress Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) as well as any other specific written or verbal information requested by referring agencies., • Provide Treatment Plans, Safety Plans, and other required clinical documents/resources/tools within 7-30 days of referral to referral agencies. Updated Treatment Plans are due no less than every 90 days to supervisor for review and sent to referral source by pre-arranged due dates., • If applicable, complete visitation/parenting time logs and send to referral sources within 48 hours of services rendered ensuring all required sections of log are compliance with current state content standards., • Oversee completion of program required assessments, including but not limited to, PFS-2, NCFAS, IARCA packets upon admission, discharge, three month and six-month follow-up., • Record all billable/direct and non-billable/indirect time on a daily basis, utilizing the established database while ensuring an accurate timesheet is maintained., • Maintain client files ensuring all required documents are in place adhering to the GJR in Indiana file checklist for each respective service being rendered. Submit and correct file audit identified issues within 7 days of receiving feedback from audits., • Document use of and adherence to evidence-based models and treatment for all services requiring use of a model that has been approved by the state of Indiana and GJR., • Obtain and submit requests for court appearances to Compliance Coordinator monthly by deadline established., • Appear at court hearings, case conferences, CFTM’s or staffing as necessary or requested by referral source or supervisor.
