Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities. STATEMENT OF THE JOB The Social Worker II is responsible for providing case management services to frail, homebound older adults, who have become unable to maintain daily living activities, making it possible for them to remain in their own homes.
