Join a mission-driven team that’s committed to putting children and families first. • Position: Outreach Representative, • Location: Jacksonville, FL, • Hours: Monday-Friday,(8a-5p) Build strong, long-term relationships with referral sources, hospitals, and clinics to drive new referrals and admissions. * Represent PHS at conferences, community events, and professional gatherings to promote our mission and services. * Collaborate with the marketing team to execute outreach strategies, campaigns, and presentations. * Identify and develop new referral opportunities based on market changes and community needs. * Conduct tours and presentations to educate healthcare professionals about our services and patient population. * Partner closely with account liaisons and internal teams to ensure seamless communication between PHS and external healthcare professionals. * Track and analyze market trends, referral data, and growth opportunities to help shape business strategy. REQUIREMENTS * Bachelor’s degree or equivalent combination of education and experience. * Minimum 3–5 years of healthcare sales, outreach, or referral development experience (pediatric home care, nursing, infusion, or nutrition strongly preferred). * Strong communication and presentation skills; able to engage diverse audiences including physicians, nurses, case managers, and community stakeholders. * Proven ability to develop strategies and achieve growth goals. * Passion for patient-centered care and a mission-driven approach to helping children and families.
