Working collaboratively with the Clinical Director, identifies educational opportunities to facilitate the ongoing development of clinicians related to the delivery of community health nursing., • Oversees & evaluates performance of individual COCs, CHNs, LPNs, related to their ability to deliver, accurately document, effectively coordinated quality driven & evidenced based care to clinically complex patients in accordance with the physician established plan of care, • Evaluates clinicians individually and collectively as a team in their ability to provide care and services in a manner that facilitates the achievement of improved patient experience of care scores as reported by Press Ganey., • Provides oversight and direction related to the scheduling of patient care services by the appropriate level of clinician to ensure high quality care delivery, continuity of care, and staff productivity.
