You need to be patient-centred and able to work with compassion with our patients and their families as they make their transition out of hospital. There is an element of case managing our most complex ‘supported’ patients through the journey of leaving hospital to ensure a safe and timely discharge. You will be a great communicator and enjoy working with the wider MDT and system partners as part of the integrated hub. You will be driven to support colleagues and advising on discharge plans across QMC and City campus.
