Job Description Job Title: CASE MANAGER Reports To: Social Work Supervisor, The Chelsea Foyer Location: 202-212 West 24th Street, New York, New York 10011 Hours:40 Hours/Week; Non-Exempt (Position requires staff to be onsite. Hybrid schedule is based on program needs and at leadership discretion) Salary Range: 50k-55k based on relevant post graduate experience Job Summary: As a member of a multidisciplinary team, the Case Manager is responsible for providing a full array of counseling, case management, advocacy services, and group work services to residents of the Foyer, an innovative 40-bed residential and vocational program for young adults preparing to transition into independent living once gaining job training, employment, and educational and life-skills competencies. Major Duties: • Oversee the diverse case management needs of approximately 14 young people experiencing homelessness funded through DOHMH., • Provide direct service, including individual counseling to residents a minimum of twice a month on site., • Work with residents to develop and implement individualized action plans and goals., • Identify appropriate resources to assist young people on their journey to independent living., • Ensure residents meet expectations of their contract and support skill-building., • Assist residents in developing recreational, social skills, and budget management., • Ensure residents comply with all program policies and procedures., • Diffuse potential crisis situations and act appropriately to resolve crises., • Assist young people in developing permanent connections with caring adults., • Provide advocacy and referrals to entitlements, shelter, legal assistance, and medical needs., • Escort residents to appointments as needed., • Participate in fire drills including climbing five flights of stairs to ensure evacuation., • Work through emergencies such as fires, gas leaks, power loss, or citywide emergencies., • Work shifts independently on evenings, weekends, and holidays., • Work with security staff to ensure appropriate visitors remain during visiting hours., • Write progress notes and maintain case records in ETO, MAVEN, or other databases., • Conduct intakes for future NY/NY3 residents including interviews and orientation., • Provide aftercare services for one year to all NY/NY3 residents moving on from the Foyer., • Collaborate with program and external staff involved in resident services., • Participate in casework, staff meetings, and trainings., • Represent the program in external meetings for resources and networking., • Participate in DOHMH site visits and follow-up actions., • Effectively use Sanctuary Model tools to support a trauma-sensitive culture., • Model the seven Sanctuary Model commitments., • Integrate the SELF (Safety, Emotional Management, Loss, Future) framework., • Bachelor’s Degree required. Two years of successful experience providing residential services to at-risk youth., • Successful collaboration with community-based agencies and experience with City systems., • Ability to communicate clearly and advocate for residents., • Flexibility to work evenings and weekends as needed., • Understanding of the needs of young adults experiencing homelessness and cultural sensitivity, including LGBTQ youth., • Commitment to strength-based, youth development, and trauma-informed perspectives., • Excellent crisis intervention, advocacy, and problem-solving skills., • Strong written and verbal communication skills., • Ability to provide first aid.
