Health Plus Management is seeking an energetic, patient-centric and detail-oriented Patient Care Coordinator to be the liaison between our physicians and patients. The Patient Care Coordinator will welcome patients and provide them the best customer service throughout their visits. In this role, the Patient Care Coordinator will educate our patients on their specific plans and respond to all inquiries with compassion. The mentor will work collaboratively with a small, dedicated team to support the client’s growth, confidence, and sense of self in the world.
