RESPONSIBILITIES Crisis Intervention • Experienced crisis hotline de-escalation, answer and follow up with hotline providing crisis intervention, safety planning, resource referrals and emotional support., • Respond to text-based outreach inquiries and crisis situations with immediate support and appropriate resource connections, • Follow-up with all hotline callers within 24hrs to ensure access to referrals and resources, • Ensure outreach and crisis intervention support services are offered with trauma-informed care, a person-centered, strengths-based, and harm reduction approach, • Facilitate access to medical, mental health, substance use facilities, housing and social services ensuring referrals are appropriate, • Conduct individualized safety plans utilizing motivational interviewing to assist survivor in identifying coping methods, proactive steps in navigating options for substance use treatment and recovery, • Collaborate with the Crisis Support Supervisor to provide feedback and updates on hotline policies and procedures to ensure quality assurance to hotline callers, • Follow-up with crisis and/or outreach responders in order to meet immediate needs and connect survivors to appropriate services through internal and external partners and community agencies, • Complete follow up with all clients who have received resources and services to ensure needs have been met and there is a comprehensive plan for next steps, • Self-regulate when experiencing triggers, trauma-responses, or stressors in order to stay engaged, productive and meet the needs of the team and clients Collaboration and Advocacy • Collaborate with REST Wellness Advocate (SUDP) for access to treatment plans, centers and recovery programs to strengthen pathways for survivors, • Collaborate with REST Outreach team and engage in direct outreach events, • Serve as a liaison between survivors and services providers advocating for survivors’ self-identified needs including addiction recovery, • Participate in multidisciplinary case reviews and community coalitions to address the intersection of sex trafficking and substance use., • Assist in educating internal and external partner on the unique needs of survivors with substance disorder and complex trauma, • Build and update resource data base for referral purposes Quality Assurance and Reporting • Conduct quarterly wellness calls on inactive and graduated clients providing emotional support and referrals and updating key impact data in client management database., • Input client related data in REST client data base, including enrollment data, goal tracking, referrals and confidential case notes., • Ensure compliance with data reporting and deadlines required for agency standards, • Complete regular documentation including client reporting, monthly reports, grant reports, financial reports, • Ensure culturally appropriate and responsive care is provided to all survivors who interact with REST, • Work collaboratively to contribute to equity, inclusion, and accessibility in service delivery and partnerships, • Participate in all necessary REST staff meetings and trainings. Job Description PCAP The Parent-Child Assistance Program (PCAP) is a 3-year intervention for pregnant and parenting women experiencing substance use disorders and their families. The goal is to help the client move along a continuum from dependence on the case manager, to interdependence with the case manager, to independence and strength on her own. PCAP’s primary aims are to assist pregnant and parenting mothers experiencing substance use disorder in obtaining alcohol and drug treatment, staying in recovery, and resolving myriad complex difficulties related to substance use; to assure that the children are in safe, stable home environments and receiving appropriate health care; to link mothers to community resources that will help them build and maintain healthy, independent family lives; and to prevent future births of children exposed prenatally to alcohol and drugs. KCRHA is responsible for unifying, coordinating, and administering the funding of emergency services for people experiencing homelessness to provide oversight and accountability. Our mission is to administer effective, performance-based homeless services to support a high-functioning homelessness crisis response system to significantly decrease the incidence of unsheltered homelessness across King County using equity and social justice principles. Crisis response includes services that support those who are not yet in permanent housing such as: Emergency shelter, Transitional Housing, Tiny Home Villages, Rapid Re-Housing, Day Centers, and Severe Weather shelter. The Level I Utilization Management Clinician performs utilization review for medical or behavioral health requests using utilization review criteria, technologies, and tools. Identifies, coordinates, and implements high quality, cost-effective alternatives when appropriate to the patient’s condition. Supports physician decision-making, working collaboratively with all members of the health care team, the patient, the patient’s family, co-workers, and internal and external customers to achieve optimal patient outcomes. Ensures members have timely access to care and supports during transitions between levels of care. Understands and effectively communicates requirements and follows Community Health Plan of Washington (CHPW) policies and procedures. To be successful in this role, you: • Have a bachelor’s degree in a relevant field or an equivalent combination of education and highly relevant experience., • Have a current, unrestricted license as an RN or LPN., • Have at least two years clinical experience in either a physical health or behavioral health setting., • Conduct review of hospital notification or prior authorization care requests against established clinical guidelines and health plan policies., • Collaborate with facilities to perform discharge planning., • Provide coordination support to members transitioning between care settings or returning home from a hospitalization. Identifies member needs and provides support to ensure necessary services are available during the transition period., • Collaborates with providers, office staff, and Care Coordination team to assure coordination of care in a timely manner according to contractual and regulatory timeframes., • Identifies, coordinates, and ensures high quality care and appropriate care by focusing on supporting access to care and services across the continuum of care in accordance with the patient’s medical needs., • Identify potentially unnecessary services and/or delivery settings and recommends appropriate alternatives., • Identifies and determines medical necessity of out of network (OON) requests for services., • Assures referrals are complete and enrollment/eligibility benefits verified, prior to authorizing care., • Delivers timely written notification to patient or family members and communicates with members of the health care team., • Prepare cases that do not meet medical necessity or criteria for medical director review., • Communicate effectively with medical director regarding identified variances within the case against criteria utilized for medical review., • Regularly communicates with the UM Manager, Medical Director, physician advisor/reviewer and primary care physician for support, problem resolution and notification of decertification and appeals., • Using established screening tools, identify candidates and recommend enrollment into care management and disease management programs., • Identify quality of care issues and report for investigation per CHPW's policy., • Participates as part of the care management team; works collaboratively with all department staff., • Reporting to work on time and for all scheduled shifts is essential to this position., • Ability to effectively manage and maintain quality standards for high volume of authorization., • Ability to work independently., • Effective written and verbal communication skills; able to communicate with and collaborate effectively with physicians and allied health care providers., • Knowledge in criteria set, including MCG, InterQual, ASAM, and LOCUS preferred., • Ability to multi-task and deal with complex assignments with competing priorities on a frequent basis., • Perform all functions of the job with accuracy, attention to detail and within established timeframes., • Effective analytical skills and the ability to interpret, evaluate and formulate action plans based upon data., • Experience in care management workflow systems., • Flexibility and willingness to work in a matrix-management environment., • Demonstrated organizational, time management, and project management skills., • Demonstrated proficiency and experience with Microsoft Office products., • Ability to present in a group setting., • Willingness to be part of a collaborative and dynamic clinical development team.
