POSITION OVERVIEW Responsible for wide range of case management duties assisting applicants and referents to Greenhope’s residential and outpatient services. Will admit new clients and assist medical staff as needed with admission documents. KEY ESSENTIAL FUNCTIONS • Primary liaison with internal and external referral sources for residential and outpatient services., • Maintains a stream of referrals to ensure residential bed and outpatient slots are covered on a weekly basis., • Complete preliminary telephone or face-to-face screening to determine eligibility for services as delineated by contractual requirements via electronic health record system.. Conducts screening interview and complete LOCADTR, WNRA, and other assessment documents.
