Job Description About this position as a Palliative Coordinator Reporting to Practice Manager, this role has primary responsibility for several key functions related to HopeHealth’s Medical Staff and Palliative Care. • Primary resource for creating, maintaining and implementing medical staff calendars. These include the Palliative Weekly Schedule, the Hospice Weekly Schedule and the Weekend and Overnight Schedule., • This person will initiate the email announcing the 3 four-month blocks for requesting ETO and ensure ETO request and approval process stays on time and on track, • Pull reports of ETO out of ADP and verify time off requests are populated to calendar, • Work with Site leaders to ensure accurate records of staff ETO and CME time-off requests, • Populate schedules in existing software and post in SharePoint and other locations, • Communicate with per diem resources to ensure adequate coverage of planned absences and assist medical staff leaders in finding coverage for unplanned ableeze as required during regular working hours., • Maintenance of accurate palliative census and support of billing functions by admitting and adding diagnosis for new hospital consults as well as discharging palliative patient charts appropriately out of myUnity. This involves critical thinking and leveraging various reports in and outside of myUnity., • Managing PING which is a system that alerts HopeHealth of patient transitions and following existing SOPs to ensure that appropriate teams are aware of patient admissions or discharges from sites as appropriate., • Supporting Medical Staff with a range of administrative tasks including submitting and tracking reimbursement requests for approval and processing., • Provide cross-coverage for other administrative resources in the Department including scheduling of patients on Advanced Illness, creation of on-call calendars, answering patient calls for community-based teams. This is a 40-hour full-time position working 8:00am through 4:30pm. Due to the need for tight and real-time collaboration, this is an office-based position based out of our Providence location. What will you do as a Palliative Coordinator Duties related to Scheduling Functions • Initiates and reminds medical staff of the ETO Blocks and Timelines, • Pulls ETO report from ADP and reconciles with appropriate medical staff leaders monthly, • Works with palliative sites lead to prepare the weekly palliative schedule, • Works with Lead HMD to prepare the Weekly Hospice Calendar, staying 1 month out that lists the weekday assignments for the Team HMDs, Hospice NP and physicians working in the Hulitar Hospice Center, • Works with CMO, Fellowship Director and Lead HMD to prepare the On Call Calendar which lists the physician responsibilities weeknights, weekends and holidays, • Posts schedules in appropriate locations, including updating and re-posting when changes are made, • Leverage software Shiftboard to assist in schedule preparation, • Assist with communication with per diem and contract staff to cover planned ETO, • Tracks CME and updates medical Staff leaders when staff use CME allowance, • Communicates areas of scheduling concern to the appropriate manager in a timely manner. Follows all appropriate protocol and guidelines for safe efficient use of resources. Duties related to Billing and Accurate Palliative Census • Admitting and adding diagnosis for hospital based palliative patients to HopeHealth’s electronic health record (MyUnity) to allow billing charges to be imported., • Determine when new palliative charts need to be created in myUnity and work with Access Center to resolve., • Closes patient charts for discharged hospital patients using PING (a patient census management system), and discharge reports from partnering hospitals, • These duties are fluid as active work is happening to automate many of these functions and may change as workflows in this area are streamlined. Duties related to ensuring continuity of care (PING) • Actively monitors patient alerts on PING and reconciles hospital-based palliative census., • Alerts appropriate palliative care team of palliative patient movements to enable real-time care coordination., • Works with HopeHealth’s medical staff, pulling reports from PING and determining discharge destination for palliative care patients so that continuity of care can be offered., • Alerts home care and hospice resources of active patient’s movement, especially to emergency room or hospital setting by monitoring the PING feed., • These duties are fluid as active work is happening to automate many of these functions and may change as workflows in this area are streamlined. Duties related to Supporting Medical Staff Dept • Relays concerns to supervisor as contribution to systems improvement., • Participates in quality improvement projects as directed., • Communicates effectively with medical staff and leaders and all other customers in a timely manner., • Identifies and participates in quality activities which support the organization’s mission, philosophy, customer service and process improvement goals., • Attends pertinent staff and team meetings and identify opportunities to improve the safe, precise and appropriate scheduling of staff., • Tracking and submitting medical staff re-imbursement requests for CME, licenses, • approval, • Consistently applies the HopeHealth values in all aspects of work, complying with established HopeHealth mission, policies, procedures, • Other duties as assigned Duties related to Cross-coverage in Dept • Assists with community-based patient and staff scheduling, calendars and answering incoming phones calls as back-up to other administrative resources in department. . EDUCATION and/or EXPERIENCE • High school diploma plus additional specialized training. Associate’s degree or graduate of a secretarial school program preferred.
