Job DescriptionBenefits: • Company parties, • Dental insurance, • Paid time off, • Parental leave, • Training & development Position Summary The Foster Care & Probation Youth Case Manager is responsible for coordinating services, monitoring safety and progress, ensuring compliance with court and DHHS requirements, and supporting youth and families involved in foster care and juvenile probation. This position includes direct client contact, documentation, court attendance, family team meeting participation, and monthly reporting. The Case Manager works collaboratively with foster parents, biological families, probation officers, schools, therapists, attorneys, and community agencies to ensure youth stability, permanency, and well-being. Essential Duties and Responsibilities Case Management & Client Support • Maintain an active caseload of foster care and probation-involved youth., • Conduct ongoing case planning and individualized service coordination., • Complete home visits with youth, foster parents, and biological families per program standards., • Monitor youth placement stability, safety, behavioral needs, and daily functioning., • Assist youth and caregivers with accessing community resources, benefits, and supports., • Provide crisis response and follow-up when placement disruptions or safety concerns occur. Court & Legal Involvement • Attend juvenile court hearings, probation hearings, and review hearings as required., • Provide accurate updates to attorneys, guardians ad litem, probation officers, and the court., • Prepare court summaries, affidavits, and documentation requested by supervisors or legal entities., • Ensure compliance with court orders and case plan requirements. Family Team Meetings / Permanency Planning • Facilitate and/or attend Family Team Meetings (FTMs), Family Group Conferences, and permanency planning meetings., • Coordinate reunification, guardianship, or independent living planning., • Support visitation plans and assist with supervised visitation coordination when required., • Document meeting outcomes, follow-up items, and participant feedback. Youth Probation Support & Accountability • Collaborate with probation officers to monitor compliance with probation conditions., • Assist youth with completing community service, school attendance requirements, curfews, and treatment plans., • Coordinate referrals for substance abuse treatment, mental health therapy, anger management, and other court-ordered services., • Provide guidance and structured support to reduce recidivism and promote positive decision-making. Documentation, Reporting & Compliance • Complete monthly case reports and submit them by agency deadlines., • Maintain accurate and timely case notes, service logs, and contact documentation., • Track case outcomes, youth progress, placement changes, and compliance milestones., • Conduct data reviews to ensure file completeness and program compliance., • Ensure all documentation meets DHHS, probation, Medicaid, and internal Remedy Road LLC standards., • Maintain up-to-date safety plans, crisis plans, and risk assessments., • Participate in audits and quality assurance reviews. Coordination of Services • Coordinate services with schools, therapists, psychiatrists, medical providers, and placement providers., • Assist youth in obtaining identification documents (birth certificate, Social Security card, state ID)., • Support educational advocacy including IEP/504 meetings and school discipline conferences., • Assist with job readiness, independent living skills, and transition planning for older youth., • Arrange transportation resources when needed and document service barriers. Foster Parent / Caregiver Support • Provide ongoing communication and support to foster parents and caregivers., • Assist caregivers with behavioral strategies, resource access, and service coordination., • Monitor caregiver needs and ensure foster homes remain appropriate and compliant., • Coordinate respite resources when available. Crisis Intervention & Safety Monitoring • Respond to placement crises, runaway episodes, behavioral escalations, abuse allegations, or hospitalization events., • Ensure required reporting is completed for critical incidents (including mandatory reporting obligations)., • Coordinate emergency placement stabilization supports., • Support youth through mental health crises and coordinate safety planning. Administrative & Professional Responsibilities • Maintain confidentiality and comply with HIPAA, state child welfare laws, and agency policy., • Participate in supervision, training, staff meetings, and professional development., • Maintain professional boundaries and ethical standards., • Participate in on-call rotation if required., • Perform other duties as assigned that support Remedy Road LLC program goals. QualificationsRequired • Bachelors degree in Social Work, Human Services, Psychology, Criminal Justice, or related field (or equivalent experience if allowed by company policy)., • Experience working with foster youth, probation youth, at-risk youth, or families involved in child welfare systems., • Strong documentation skills and ability to meet deadlines., • Ability to pass background checks required for work with minors., • Valid drivers license and reliable transportation., • Ability to travel locally and statewide within Nebraska as needed. Preferred • 13+ years case management experience in foster care, juvenile probation, or behavioral health., • Experience working with Nebraska DHHS, juvenile courts, or probation services., • Knowledge of trauma-informed care, behavioral interventions, and family systems., • Bilingual abilities are a plus.
