The Primary Care Physician (PCP) works as a lead in our team-based care environment. We are a value based care provider focused on quality of care for the patients we serve. Our care team consists of doctors, advanced practice professionals, Pharm D, care coach nurses, MAs, behavioral health specialists, quality based coders, referral coordinators and more. Our approach allows us to provide an unmatched experience for seniors. Our model is positioned to provide higher quality care and better outcomes for seniors by providing a concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships. This robust support allows our PCP to see fewer patients and spend more time with those they do. * $60,000 Sign-on Bonus * Total compensation package (base pay + bonus) could exceed $300K depending on experience and location. Responsibilities: * Evaluates and treats center patients in accordance with standards of care. * Follows level of medical care and quality for patients and monitors care using available data and chart reviews. * Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care. * Acts as an active participant and key source of medical expertise with the care team through daily huddles. * Helps Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor. * Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity. * Follows policy and protocol defined by Clinical Leadership. * Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues. * Participates in potential growth opportunities for new or existing services within the Center. * Participates in the local primary care “on-call” program of CenterWell as needed. * Assures personal compliance with licensing, certification, and accrediting bodies. * Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care. Required Qualifications: * Current and unrestricted medical license or willing to obtain a medical licenses in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required * Graduate of accredited MD or DO program of accredited university * Excellent verbal and written communication skills * Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients * Fully engaged in the concept of “Integrated team based care” model * Willingness and ability to learn/adapt to practice in a value based care setting * Superior patient/customer service * Basic computer skills, including email and EMR * This role is considered patient facing and is a part of CenterWell’s Tuberculosis (TB) screening program. Primary Responsibilities • Provide all athletic training and assist with medical support for the ENT as requested by the Chief Medical Officer, Director of Medicine and Sports Medicine Senior Manager, • Travel periodically with ENT to training camps and games both within the United States and abroad, • Responsible for the design and delivery of U.S. Soccer standards in injury prevention, recognition, evaluation, treatment and rehabilitation to all ENT players, • Responsible for evaluating and referring significant injuries and/or illnesses to the team physician, consultants, hospitals or specialists as appropriate, • Responsible for tracking and developing individual and team programs, specifically injury mitigation, mobility, and rehabilitation as needed with all ENT players within U.S. Soccer camps and within their club environments, • Ensure all medical data collected, is stored and up to date correctly in line with U.S. Soccer standards in AMS and EMR platform designated by U.S. Soccer, including but not limited to; nature of injury or illness, treatment provided, date, insurance, injured reserve, injury protection, medical authorization, medical information release, physical examinations, medical referrals, surgical reports, and x-ray/MRI information, • Ensure all physical programming is designed and stored in line with U.S. Soccer standards, • Ensure all pre, post, and in camp data collected is disseminated to all required staff and players in line with U.S. Soccer standards and time frame, • Work closely with the sports medicine and sports science staff to ensure U.S. Soccer standards in all testing, screening, monitoring, and return to play protocols are optimized, delivered, and recorded, • Assist Sports Science staff in camp to support athletes and inform/guide coaches on best practices to optimize performance and health of athletes, • Collaborate with Sports Science staff on training and injury prevention strategies outside of camp, • Maintain medical inventory, supplies, and equipment and evaluate new technology/devices to support athlete care, • Build a strong relationship with ENT players/clubs domestic and abroad to ensure a bi- lateral sharing of data and information is provided pre and post-camp, • Assist in training and supporting other medical staff within ENT training camps, • Assist in the development and support of all athletic training practice across U.S Soccer, • Other duties as required or assigned, which may include:, • Help support and develop internal and external continued professional development for staff within the federation and practitioners within clubs, • Contribute to the education of staff and coaches within the federation, and those enrolled in federation specific qualification pathways, • Support and develop athletic training practice within the college and/or other soccer institutions, • Participate in partnership with IF/THEN and STEM initiatives, • All duties listed above with other National Teams (Youth, Extended) as needed Requirements: Minimum Qualifications • Master’s degree in Athletic Training or related field, • NATABOC certified athletic trainer (AT), • Member of NATA or similar governing body, • Current CPR/AED for the Professional Rescuer (or equivalent) certification, • Minimum 5 years of clinical experience working within Soccer and/or other elite sport, • Minimum of 5 years' Experience working with Cerebral Palsy (CP)/Paraplegic players/athletes, • Capable of working in fast-paced and demanding environments, • Must be able to connect “micro” details to the “macro” vision and mission, • Proficient experience with Microsoft Suite (Word, PowerPoint, Excel and Outlook), Electronic Medical Record Systems, GPS System or other player monitoring platforms, • Able and willing to travel up to 50% of the year, • Able and willing to work non-traditional hours including evenings and weekends as needed, • Must be able to stand for long durations, squat, bend, lift, carry, and climb ladders, • Passion for soccer, • Physical Therapist with experience special needs players/athlete, • Proficient in managing and executing emergency medical care best practice, • Advanced education in manual therapy and rehabilitation techniques, • Advanced certification/experience in applied movement analysis and biomechanics (AFS or related), • Experience working in collaboration with coaches and other support staff, • Proven track record in delivering world-class medical provision to elite players, • Vast knowledge and expertise of medical modalities, equipment, and technology, • Bilingual (English and Spanish), • Thorough institutional knowledge of U.S. Soccer’s mission, goals, technical plan, policies, practices, and procedures, • Previous experience with a club or national team, • Working at U.S. Soccer is a unique opportunity. Duties and Responsibilities • Understand federal, state, county and local laws regarding shelter administration, • Independently manage day-to-day operations, oversee staff and volunteers, hiring and training, evaluating performance and setting schedules, • Responsible for delegating work tasks, addressing employee and volunteer concerns and enforcing policies, • As a partner with the Medical Director, oversee general and medical care of shelter animals, ensure humane treatment and determine when animals require veterinary care, • Monitor financial statements and adjust spending as necessary to remain within budget, • Ensure that policies and procedures of POA’s manuals are followed and updated as needed, • Developing job descriptions, protocols, and documentation related to shelter operations, • Prepare a monthly report for the Board of Directors and attend monthly meetings, • Ensure accurate and complete records for all dogs, staff and volunteers, safety policies, and facility maintenance are maintained as required, • Supervise and evaluate shelter staff, contracted veterinarian and volunteers to provide for and monitor medical care, behavior assessments and progress of dogs in the shelter, • Set standards for and monitor the conduct of shelter employees to ensure a humane philosophy is projected to the public and implemented in the care of all animals, • Manage new and on-going shelter projects to determine conformance and implement changes due to budget requirements or external factors, • Participate in planning and coordination of community outreach, education and fundraising events, • Develop and maintain positive, professional relationships with the public, staff, volunteers, local veterinarians and animal rescue organizations, • Manage media information programs to ensure that the public is provided accurate information and friendly service, • Proactively respond to and handle complaints about staff, animal care, and iWag policies; professionally resolve difficult situations, • Provide accurate information about adoptable animals, ensuring our web-based list of adoptable animals is kept up-to-date, • Review sampling of adoption agreements and animal records for accuracy and completeness, • Assess the general health of shelter residents on a regular basis for signs of stress, illness, injury, aggressive tendencies and abnormal behaviors. Partners with Medical Director and Contract Veterinarians to establish proper medical treatment and training needed based on these assessments, • Establish protocols for daily care to include proper feeding, watering, sheltering and monitoring for signs of disease, injuries, and abnormal behavior.
