Job Description GENERAL PURPOSE Under the administrative supervision of the Program Director or designee, this position is responsible for completing initial and annual assessments, providing psychosocial, rehabilitation and recovery-oriented services for adults. DISTINGUISHING CHARACTERISTICS This is an at-will direct service position within a program. This position is responsible for ensuring that clients are introduced into the program in a comfortable, thorough and timely manner. This position reviews the initial assessment of clients to confirm eligibility under target population and geographical boundary criteria. This position is also responsible for ensuring that annual re-assessments are completed in a strengths-based, client-centered manner and that the assessment and treatment plan are shared with the multi-disciplinary team working with the client. ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY) The duties listed below are intended only as illustrations of the various types of work that could be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to this class. • Responsible for assisting members in identifying, expressing, asserting and communicating their goals and needs while living in the community, • Responsible for completing the intake and renewal Core Assessment, including collateral assessments (AHQ, MSE) in the field or in the office in coordination with the client’s multi-disciplinary team, • Responsible for facilitating the development of the Individual Client Plan in collaboration with the member and the treatment team, • Provides on-going consultation to the treatment team and other staff to support the delivery of services that are driven by the member’s client plan and supportive of the member’s goals, • Completes all required paperwork to include annual member paperwork as assigned in a timely manner, • Drives on program business using personal vehicle for the purposes of outreach and engagement, assessments, medication outreach, transporting clients to appointments/activities, community/county meetings/trainings, • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation, • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program, • Participates in off-site trainings and provides in-service training for all staff, • Conducts Quality Management activities as directed by the Clinical Director and Team Leader, such as preparation for county Utilization and Review audits, internal audits, review of services provision and charting, and other projects as assigned, • Facilitates groups, provides individual and family therapy, and updates diagnoses for clients not receiving medication management services from the program psychiatrist, with oversight from a licensed professional if not themselves licensed, • Carries a small caseload, • Attends to the safety, health, and wellbeing of clients, • Completes paperwork as assigned in a timely manner, and maintains productivity standards outlined for the program Education, Training and Experience A typical way of obtaining the knowledge, skills and abilities outlined above is by having an AA degree and 6 yrs of relevant experience or a Bachelor’s Degree and 4 yrs experience or a Masters Degree and 2 yrs of relevant experience preferably working with ALL demographics, populations, and ages (TAY, adults, and older adults). Experience in conducting functional assessments, bio psychosocial assessment, and other clinical approaches in interviewing and gathering data based on mental health needs, diagnosis, and functional impairments. The Program Director for the SIERRA ELDER WELLNESS PROGRAM (SEWP) Outpatient Mental Health Program is responsible for overseeing and managing all aspects of outpatient mental health programs specifically designed to meet the unique needs of older individuals. Job Description POSITION TITLE: Adjunctive Therapist (CTRS, OTR, or ATR) REPORTS TO (TITLE): Director of Clinical Services Schedule: Per Diem/On call (Weekends, but flexible to include weekdays and holidays, etc.). Responsibilities • Works with RVP of Sales in evaluating and qualifying new business opportunities as to profitability and strategic impact on the pharmacy and the region in order to achieve annual revenue goal, • Works with RVP of Sales in identifying business opportunities through sales calls, referrals, targeting reports and CRM and in communicating (verbally and in writing) with IDD agencies and behavioral health organizations to assess market needs, • Works with RVP of Sales in finalizing contracting, coordinating startup of new account and transitioning account services to pharmacy operations, upon obtaining service commitment from client, • Works with RVP of Sales to utilize marketing segmentation information in developing strategic territory plans and executes those plans to budget time effectively and target high potential prospective customers, • Works with RVP of Sales ,local Pharmacy Director and Account Manager to maintain and strengthen customer relationships by providing ongoing support and value to customers through products and services, • Responsible for customizing IDD and BH presentations and proposals to meet the customers needs, • Director of Business Development is responsible for attaining quarterly and annual total bed and revenue goals, • Communicates with other pharmacy team members (Pharmacy Consultant, and pharmacy staff) to uncover leads and resolve customer service concerns, • Completes sales, reporting, and contracting for new business and other administrative procedures as determined by the RVP of Sales in a timely and accurate manner, • Successfully completes all components of sales training and implements sales process and procedures, such as pre-call planning, into everyday practice, • Participates in local, state and national industry trade shows as required to grow area of responsibility, • Performs other tasks as assigned, • Conducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct and Ethics, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards, • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily, • The requirements listed above are representative of the knowledge, skill, and/or ability required, • Each essential function is required, although reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions Qualifications • Must have a Bachelor's Degree or equivalent experience, • Experience selling healthcare services to IDD agencies and behavioral health facilities is preferred, • Three or more years of prior sales experience background required in healthcare sales, medical equipment sales, consumer products and services/solutions sales experience a plus, • Experience selling and negotiating contracts for products and services a plus, • Proficient computer skills in MS Office Suite including: MS Word, MS Excel, MS PowerPoint, and MS Outlook, Hubspot and CRM., • Must have valid driver's license and willing to travel 75%., • Must be results orientated, adaptable, product/service knowledgeable, organized, and have good oral communication skills
