Job Description JOB SUMMARY The Placement coordinator is responsible for all placement coordination efforts for Muckleshoot children in need of placement through the Muckleshoot Children and Families Services (MCFS) program. This position serves the Muckleshoot Children and Families Services program as a whole. This position will work collaboratively with the MCFS Foster Home Licensor in assisting recruitment of foster placement within the Muckleshoot community. MAJOR TASKS AND RESPONSIBILITIES • Coordinate with department Family PAL’s and other State and Indian Child Welfare (ICW) caseworkers to provide assistance, and support for the emergency out-of-home placement of Muckleshoot children., • Perform annual foster home health and safety visits ensuring compliance with licensing regulations and policies. Documents visits, noting any need or deficiencies and offer solutions for any concerns founded with in the health and safety visits., • Work directly with the licensor in the writing of foster care home studies and will assist the licensor in maintaining compliance of the homes that are licensed under the Muckleshoot Tribes’ Child Placing Agency (CPA) license., • Work with Muckleshoot Children and Family Services Family PALs to coordinate child placement by maintaining accurate and up to-date records of home placement availability. Ideal candidates will have strong triage, telemetry, and critical care skills, with experience managing ventilated patients and responding to stroke and STEMI alerts. Prior EPIC EMR experience and TNCC certification are preferred. What you'll likely work on • Provide best in class comprehensive primary care in an outpatient setting., • Partner with the care team and leverage One Medical Seniors coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change., • Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population., • State of the art practice setting designed for a team-based practice model, • Leadership that values your input and understands the importance of work/life balance, • Weekly protected time during the work day, to gather with fellow One Medical Senior providers for team building and shared discussion of challenging cases.What you'll need:, • At least 2 years of primary care experience required., • Currently licensed or ability to obtain licensure in the state of Washington., • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups., • Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes. The base salary range for this role is $286,900 to $304,980 based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule.
