Job Description Esperanza Immigrant Rights Project, a program of Catholic Charities of Los Angeles, Inc., is a non-profit law firm which provides community education and legal representation to immigrant adults, families, unaccompanied minors, individuals with mental health issues, and in cases involving crimmigration. Esperanza has proudly provided legal services to immigrants and their families in Southern California for over 15 years. The immigration court system does not provide court-appointed counsel to individuals facing deportation who cannot afford an attorney, so this vulnerable population relies on our community education programs to obtain critical information about the process, understand their legal options and access assistance. Job Description Location: Downtown, Los Angeles CA Position Purpose: Under the direction of the Program Manager, the Care Team Specialist will be participating in a multi-disciplinary team, contributing to client engagement, retention, safety, and support. The position will interact with staff and clients as a care team member by building rapport and fostering a sense of community. The Pathway Home program is an Interim Housing program that assists homeless, individuals to recover and improve their quality of life. These individuals may also suffer from mental health and substance abuse issues, sometimes causing behaviors such as yelling, cussing, and outbursts of anger. The role of each CTS staff at DTLA is to assist with safety of each residence. Each CTS during shift is expected to use our electronic tagging system to check all perimeters of the building including the top floors. Program Population: The Pathway Home Housing Project is a HFH funded program, operated by JWCH. The Team consist of a cohesive team of MSW, case managers, site monitors and other professionals who collaborate to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. The clients/residents may come with behavioral and/or substance abuse issues. The program utilizes the Harm Reduction Model and we “do whatever it takes” to encourage success of the client. Job Description Location: Downtown Los Angeles, CA Job Type: Full-Time / Part-Time Job Summary: We are seeking dedicated primary care/addiction medicine physicians to join a multidisciplinary, inter-agency team at two multi-service Harm Reduction Health Hubs in Skid Row, Los Angeles. These hubs are designed to provide holistic care for people experiencing street homelessness and who use drugs. The physician will be primarily responsible for direct patient care but will also have the opportunity to take on a leadership role in implementing clinical programs aimed at reducing overdose and other drug-related harms, sexually transmitted and other communicable diseases, harms related to interpersonal violence, mental health crises, self-harm, and accidental injury. The physician will provide transitional primary care, urgent care, and substance use disorder-related care in a low-barrier, open-access outpatient clinic and a ~20-bed short-term community respite space. On-site clinical and social services are rooted in the ethos of harm reduction and trauma-informed care. Candidates should demonstrate a commitment to providing non-judgmental, patient-centered care to marginalized communities and individuals with complex medical and psychosocial histories, including people experiencing homelessness, those who use drugs, engage in sex work, and/or have been incarcerated.
