An MLTC assessment nurse is responsible for evaluating pre-screened individuals for eligibility in Managed Long Term Care programs. The nurse conducts in-person interviews at the patient's home, engaging directly with the patient or their guardian. Responsibilities: - Evaluate Medicaid members for eligibility in managed long-term care using a comprehensive web-based platform. - Perform initial and annual assessments remotely, adhering to state guidelines, policies, and protocols. - Collaborate with healthcare providers and review medical records and medications to ensure accurate assessment outcomes. - Revise and update assessments as needed in accordance with company policies.
