HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Mission Hills, California. • Specialty: Case Management, • Discipline: RN, • Start Date: 12/01/2025, • Duration: 18 weeks, • 39 hours per week, • Shift: 8 hours, days, • Employment Type: Travel Unit Type: • Typical caseload: up to 20 patients per day, • 8-hour shifts: 08:30 AM – 05:00 PM, • Schedule options: Sun–Thurs or Tues–Sat, based on department needs, • Weekend coverage required per staffing demands, • Floating required, • Perform comprehensive assessments, determine needs, and drive care coordination, • Lead all aspects of discharge planning, including DME, SNF placement, home health, and ambulance arrangements, • Conduct admission reviews (InterQual – ED only) and continued-stay assessments, • Coordinate care for complex populations including:, • Patients experiencing homelessness, • Substance use disorders, • Behavioral health needs, • Patients needing POLST/advance directives, • Protective services involvement, • Organize and participate in family meetings, • Collaborate on treatment planning and resource alignment, • Work effectively with HMOs, IPAs, insurance partners, • Minimum 2 years acute-care hospital Case Manager experience, • MUST include care coordination + discharge planning, • Behavioral health–only backgrounds are not accepted, • Ability to independently manage all discharge tasks including DME, SNF, and transport, • Strong knowledge of regulations, benefits, medical necessity, and utilization review, • Must be proficient with:, • InterQual/Milliman, • CMS regulations, • DRG principles, • HIPAA, • HEDIS, • Joint Commission/core measures, • Worker’s Compensation workflows, • Proficiency with Microsoft applications required, • Admission, concurrent, and continued-stay reviews, • Medical necessity assessment, • Prior auths / pre-cert, • Appeals & denials, • Retrospective reviews, • Quality improvement participation, • Care planning and DME coordination, • CPT/ICD-10 familiarity, • Data abstraction, • Social Services, • Pharmacy, • Physical Therapy, • Respiratory Therapy, • Radiology, • Transportation, • Lift teams, • IV teams, • Rapid Response teams, • Dedicated hubs for:, • Discharge planning (home health, SNF search), • Travel experience required; first-time travelers may be screened, • Orientation: ~40 hours, including 3-day unit-specific training, • Scrubs: Not provided (no color requirement listed) HonorVet Technologies: Pioneering Purpose, Empowering Heroes Founded in 2015, HonorVet Technologies stands as a Service-Disabled Veteran-Owned Small Business (SDVOSB), proudly certified by NaVOBA and the U.S. Department of Veterans Affairs.
