Job Description Woodbridge Trifari is looking for a Registered Nurse (RN) to implement, coordinate, and supervise the provisions of in-home support and care needs of our clients. PRE-EMPLOYMENT REQUIREMENTS: • COVID-19 test, • Criminal background check PHYSICAL REQUIREMENTS: • Ability to lift up to 15 lb., • Ability to reach, bend and walk., • Ability to comprehend complex materials, • At least 21 years of age, • Strong supervisory experience and leadership skills, • Preferably with experience in acute care (at least one year) and planning, implementing and evaluating IHCP (at least 2 years) required. Independence in clinical practice., • Clinical and work experience related to Adults and older adults with developmental disabilities and Long-term Care and Home health experiences preferred., • Ability to work within the individual health care team with initiative, imagination, resourcefulness and flexibility., • Computer literacy., • Cultural competency and the interest and ability to work within a multilingual, multicultural environment., • Must meet the initial health screening that includes Tuberculosis screening., • Ability to communicate and collaborate with client’s family members and health care providers., • Completed background check / fingerprinted., • Current First Aid and CPR, • Participates in the Individual Health Care team responsibilities of care assessment, planning, and management with focus on in-home/community needs of the clients including: initial and ongoing assessment of home environment in relation to changing needs of the clients, • Coordinates information from clinic, day health center and home care staff, and family members in the provision of a IHCP or Individual Service Plan, • Participates in the Administrator-On-Call rotation, • On duty and physically present at least 32 hours per week to ensure effective operation of WT Home, • Actively participates in ensuring that Woodbridge Trifari meets the needs of the clients including admission, screening, transfer assistance and discharge planning, • Acts as advocate in ensuring that clients are receiving medical care, including working with IDT, consultants, and primary care physician in ensuring that the client’s medical needs are met or communicated in a timely manner, • Provides on-going assessment in client's home and, in coordination with Inter Disciplinary Team, client’s primary care provider in developing and implementing follow up care and services as indicated in the client’s treatment plan, • Responsible for the development and implementation of competency-based initial orientation and on-going education of licensed staff and direct care providers, • Functions as a member of Woodbridge Trifari facility management team to promote leadership and program operations., • On-site supervision and instruction of staff (licensed staff and direct support professional) in client’s home and community settings, • Actively participates in the admission and discharge process, • Ability to work with culturally diverse direct care providers, • Ensures appropriate documentation for consistency with the IHCP, • Meets all the administrative requirements including statistical records, medical records, utilizing computerized documentation system, attending staff meetings, etc, • Participates in multidisciplinary team /family meeting to meet Regional Center, DDS and Community Care Licensing guidelines and standards in complaints, incidents and grievance, • Participates in Woodbridge Trifari committees, such as Infection Control, Utilization Review, Quality Improvement, and Safety Committee, • Actively participates in and encourages actions that promote good public relations with clients, their families, visitors and the community
