Job Description SUMMARY: The Behavioral Health Provider I is responsible for delivering comprehensive behavioral health services within an integrated primary care setting. This includes conducting assessments, developing treatment plans, providing short-term interventions, managing patient care, and collaborating closely with primary care providers to address patients' mental health, substance abuse, trauma, and psychosocial needs. The role requires regular on-site presence to ensure seamless integration with the healthcare team. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES: • Supports and implements Gracelight’s vision, mission, and values; fosters and promotes a culture of service excellence and accountability., • Performs all job functions in a professional and courteous manner, including timely answering of phone calls and emails and providing excellent customer service to internal and external customers., • Delivers behavioral health services in an integrated primary care environment, working collaboratively with medical providers to offer holistic, patient-centered care., • Conducts functional and strengths-based bio-psycho-social assessments along with comprehensive diagnosis for mental health, substance abuse, trauma, and psychosocial needs., • Utilizes both short-term interventions, as approved by the funding source, targeting trauma, mental health symptoms, and reduction of substance use., • Consults with primary care providers to enhance understanding of the patient, provides support for treatment approaches that incorporate behavioral health needs, and assists with implementation of bio-psychosocial treatment and care plans., • Collaboratively develops multi-disciplinary workflows, clinical pathways, and other practices to maximize the benefits of team care for patients., • Responds promptly to primary medical care staff for crisis intervention during psychiatric emergencies and assists staff during any other type of emergency., • Works alongside primary care providers for warm hand-offs, connects with medical staff, and provides brief short-term interventions to patients in need and/or crisis., • Coordinates and manages patient care, ensuring continuity and quality of care. Follows up with patients to monitor progress, monitors adherence to treatment plans, and addresses any barriers to care., • Maintains daily visit targets to ensure optimal patient care and service delivery., • Actively participate in monthly clinical supervision and further enhance professional growth through training and consultation with other paraprofessionals., • Completes all charting in a timely manner and close charts within 72 hours, including assessments, progress notes, care plans, and billing documentation., • Complies with organizational policies and procedures and adheres to HIPAA regulations. Follow reporting protocol for both adult and child clients., • Maintains a high level of professionalism and adherence to organizational policies., • Demonstrates respectful and collaborative behavior with colleagues, patients, and supervisors., • Supports client involvement in planning, implementing, and evaluating services., • Completes data collection forms and prepares reports as required., • Provides clinical supervision to Behavioral Health II staff members as needed., • Participates in the Comprehensive Perinatal Services Program (CPSP) and adheres to clinical quality measures related to prenatal and postpartum depression screenings., • Conducts prenatal and postpartum depression screenings and follows up on positive depression screenings to ensure appropriate care and support., • Maintains a comprehensive list of resources to assist patients with their practical, social, and emotional needs., • Demonstrates flexibility to change and to evolving healthcare needs, organizational changes, and advancements in the field., • Must be willing and able to work at all locations as needed to meet patient care needs., • Must be willing and able to work all business hours including evenings and weekends. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Master’s degree in social work, psychology, counseling, or a related field., • Licensed or license-eligible in the state of employment., • In good standing with the California Board of Behavioral Sciences with an active license., • Experience in behavioral health, preferably in a primary care setting., • Strong diagnostic and clinical skills., • Possesses excellent written and verbal communication skills, utilizing, tact, active listening, empathy, and compassion. Able to manage conflict in a direct and professional manner., • Ability to work effectively in a team-based care environment., • Proficiency in electronic health record systems and documentation., • Experience working with adults, children, and families from diverse social, cultural, educational, and economic backgrounds., • A genuine passion and deep appreciation for the mission of the Organization., • Valid driver’s license with current automobile insurance., • Current CPR certification required., • Ability to organize and prioritize work tasks., • Proficiency with computer applications such as Microsoft Excel, Word, and PowerPoint; EPIC preferred but not required., • Bilingual (Spanish, preferred)., • Strong problem-solving and critical thinking skills., • High level of cultural competence and sensitivity., • Flexibility and adaptability in dynamic work environments., • Strong organizational skills and attention to detail.
