Description: Milton Hershey School (MHS) is a top-notch home and school where 2,000 pre-K through 12th grade students from disadvantaged backgrounds are provided an extra-ordinary cost-free career focused education. This is made possible by the generosity of Milton and Catherine Hershey, who established the school in 1909 and ensured it was fully endowed. Thanks to that foresight, the school has almost 12,000 graduates and is expanding to serve more students. MHS is hiring Flex Transitional Living Assistants (FTLA) to live with and supervise high school seniors in campus residence buildings in Hershey, PA. FTLA’s mentor MHS seniors to ensure they develop the life skills necessary for success post-graduation – whether they are entering post-secondary education, the military, or the world of work. FTLA's prepare students for the challenges they’ll face by role-modeling life skills and teaching self-responsibility, goal setting, planning & prioritization, time management, personal finance, and interpersonal skills - including appropriate dating behavior and managing peer pressure. Other responsibilities include creating and supervising evening & weekend recreational activities, taking students to Sunday chapel services, and collaborating with fellow staff members to continually evaluate and enhance the program. Additionally, FTLA’s provide coverage in MHS’s students homes as needed. This is a year-round position, working a schedule of 9 consecutive days, followed by 3 days off. Compensation includes a minimum annual salary of $48,799 plus a benefits package. Housing & utilities are provided. Up to $2,800 in relocation assistance is available. Ideally, the Medical Support Specialist will be familiar with regulations of the plasma collection industry and/or a manufacturing environment. The Medical Support Specialist follows guidance provided by BioLife Medical Affairs and provides center level support of environmental, health and safety (EHS). ACCOUNTABILITIES • Donor Eligibility (85%), • Determine donor eligibility accurately and in a timely manner, to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE) as required., • Evaluate and manage donor reaction(s), which occurs at the facility as outlined in the SOPs. Primary Duties and Responsibilities • Develops, implements, coordinates, reviews and updates case management and individual service plans for ISAP participants in conformance with state and federal regulations and program requirements. JOB SUMMARY: As a key member of the SkillSpring (f/k/a GCD) department, the Workforce Programs Case Coordinator will engage with current participants in the young adult and high school programs in an effort to prepare for and secure employment. The role of the Workforce Programs Care Coordinator is to collaborate with SkillSpring participants using the care management process including: assessing a person’s functional level, their physical, cognitive, social-emotional status and their support system; developing a plan of care that addresses the needs and problems identified and incorporating the services that are needed to enhance the current support system; identifying and arranging for coordinated delivery of those services; monitoring changes in the person’s condition, and reassessing the person’s needs on a regular basis. This team member will serve in a coaching role and effectively advocate for program participants to have their various needs met with compassion and grace. REPORTS TO: Young Adult Program Manager- Workforce Programs DUTIES AND RESPONSIBILITIES: 1. Ensure the health, safety and well-being of participants in the young adult program, by providing support and supervision of all activities., 2. Screen and identify young adult participants in need of care coordination services. Conduct comprehensive assessments by interviewing young adult participants to identify their participant’s needs and goals., 3. Collaborate with the SkillSpring Young Adult or Health Care Explorers teams, to identify problems or needs that require special planning, intervention, teaching or follow-up., 4. Actively support measures that prioritize the participant’s needs and promotes the effective use of resources. Facilitate the participant’s involvement in the direction of care., 5. Identify, plan and arrange for appropriate services by applying a knowledge of local regional and federal services and regulations. Collaborate with the participants, programming teams and community services., 6. Coordinate implementation of the care plan with case conferences and working collaboratively with community based partners on these conferences. to maximize continuity of care, 7. Monitor the participant’s needs and responsiveness to care plan interventions. Communicate with providers who are delivering care and services for the purpose of maintaining the quality of care. RESPONSIBILITIES • Provide trauma-informed, voluntary case management services to clients and their families to meet the overarching program goal of maintaining permanent housing and achieving a higher level of independence., • Manage a caseload of approximately 40-50 families – three to six months being the average length of an open case., • Conduct 50 face-to-face client contacts each month which will be a combination of clients on site in the last month of their shelter stay and those out in the field who moved out into permanent housing., • Develop, review, and adjust service plan collaboratively with the client covering the program’s primary service areas: finances and housing, education, employment, health and mental health, and legal (as needed).
