Job Title: LPN Team Case Assistant STATUS: Full Time LOCATION: Rochester, NY DEPARTMENT: Home Care SCHEDULE: Days SUMMARY The LPN Team Case Assistant will utilize clinical experience, and coordinate patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion. Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met.
