Provides general nursing care to residents in the retirement community and health center by performing the following duties listed below. ESSENTIAL DUTIES AND RESPONSIBILITIES • Connect residents and families to necessary resources and supports in the community., • Encourage residents to participate in healthy activities, example; Edgemere Masterpiece Living., • Perform Resident Psycho social assessments when needed, • Assist /Educate residents and families to participate in decision making when there is a change in health condition., • Help residents/families adjust to admission to Plaza Health care services; possible role changes; exploring emotional/social responses to illness and treatment of a resident., • Educate residents and families on the different levels of health care available; entitlements, community resources and advanced directives., • When needed perform health screening and evaluation of new and current residents., • Follow up with incident reports, explore possible ways of preventing future such incidents., • Work closely with Home Health Providers and assisting residents with coordination of additional services within their apartments., • Enforce the Caregiver Handbook., • Ensure residents are receiving Quality care from caregivers, Follow up with complaints.
