Job Description Position Summary: Veterans Place of Washington Boulevard is seeking a Recovery Support Case Manager, a critical member of the RSS program, providing comprehensive care coordination for veterans with SUD, mental health conditions, and housing instability. This position links veterans to internal and external resources, monitors progress and serves as a consistent support throughout the recovery continuum. Key Responsibilities: • Conduct intakes, initial assessments and develop detailed Recovery Plans using SMART goals., • Coordinate care plans addressing housing, healthcare, legal services, workforce development, and recovery., • Refer veterans to other Veterans Place programs and external partners for community-based services and track progress on goals., • Maintain up-to-date documentation in BestNotes and PA-WITS in line with DDAP and internal expectations., • Participate in clinical meetings, case reviews, and program evaluations., • Coordinate with CRSs to ensure veterans are receiving holistic care from RSS., • Support transitions between programs (e.g., from THP to permanent housing)., • Facilitate crisis intervention and connection to detox/residential/outpatient treatment as needed., • Ensure that the Recovery Oriented System of Care (ROSC) concepts and guidelines are incorporated into services provided., • Record interactions and progress in the BestNotes system following HIPAA and DDAP documentation standards., • Collaborate with other Veterans Place programs and partners to support integrated care., • Provide wrap-around support by coordinating access to Veterans Place services (housing, treatment services, recovery support, healthcare, etc.). Key Competencies: • Ability to build rapport with vulnerable clients and maintain professional boundaries, • Strong organizational, documentation, and crisis management skills, • Commitment to person-centered, recovery-focused service planning, • Cultural competency and ability to engage with diverse veteran populations Minimum Education & Certifications: • Bachelor's degree in social work, Counseling, Psychology, Human Services, or related field. (Equivalent experience may be considered), • Minimum 2-3 years of experience working in case management, homeless services, behavioral health, or recovery settings and with care coordination, • Familiarity with ASAM, trauma-informed care, and motivational interviewing, • Experience working with recovery populations, veterans, or peer support programs preferred., • Strong organizational skills and ability to focus in high stress situations Additional Qualifications: • U.S. military veteran preferred., • Preferred licensed or credentialed in PA (e.g., LSW, CADC, etc.), • Must have a valid driver's license., • Basic proficiency with Microsoft Office and digital platforms. Cognitive and Sensory Requirements: • Ability to focus on Recovery Support program details, follow through with timelines, and attend to multiple tasks and interactions., • Ability to retain and recall information regarding clients, policies, procedures, and schedules., • Capable of identifying issues, evaluating solutions, and making appropriate decisions independently or collaboratively., • Ability to comprehend written and verbal information, express thoughts clearly, and communicate effectively with diverse populations (e.g., staff, veterans), • Capacity to manage time effectively, maintain accurate records, coordinate meetings., • Adequate visual acuity (with or without corrective lenses) to read documents, use a computer, and observe participants in meetings or events., • Adequate auditory ability (with or without assistive devices) to understand conversations in person, over the phone, or in virtual meetings., • Clear verbal expression to articulate expectations, deliver feedback, and conduct presentations Physical Demand Statements • Must be able to lift, carry, and move materials weighing up to 50 pounds regularly using proper lifting techniques., • Required to stand or walk for 6 to 8 hours daily on concrete surfaces., • Ability to sit and use computer for extended periods (4+ hours) with breaks., • Job functions include working outdoors in varying weather conditions including heat, cold, and rain.
