Job Description For 70 years, Light of Life Rescue Mission has been a safe refuge for homeless and hurting people in the Pittsburgh area. We are currently seeking a full-time Care Team Leader - Life Recovery Program. This person will walk closely alongside clients to help them become self-aware of issues while creating a loving, grace-filled environment for them. Light of Life is committed to building a staff community that reflects the diversity of His creation. ~ Jerrel Gilliam, Executive Director Responsibilities include but are not limited to: • Provides case management and oversees daily operation of programming for clients in the Life Recovery Program., • Tracks clients' Care Plan progress through scheduled, weekly one-on-one sessions and data entry into MissionTracker database., • Ensures complete and accurate data entry into MissionTracker client database for clients assigned to case load., • Conducts weekly inspections of Life Recovery Program and/or Mental Health Program living quarters and communal spaces to ensure compliance with program guidelines., • Administers weekly, random urine screenings to clients enrolled in Light of Life's Men's Residential Programs in collaboration with on-site laboratory technician and MRP Care Team., • Networks with relevant and/or related service providers to access the services needed for clients and ensure delivery., • Facilitates and develops curriculum for Men's Residential Program groups/classes/, • Has veto authority over any proposed activity that does not follow Light of Life policies and/or is deemed unhealthy or harmful to recovery., • Exercises input authority into MRP Care Team decisions during weekly continuum of care meetings., • Exercises communication authority over decisions affecting the policy or procedures of the Life Recovery Program., • Walk alongside clients as a disciple of Christ and share the Gospel of Jesus Christ with them through actions and words., • Establish mutual trust and create a safe, authentic environment so that the clients continue to mature in Christ., • Serve the clients in a Christ-like manner and practice a self-evaluative lifestyle., • Resolve all issues in ways that promote the principles of the healing environment while following the processes described in the LOL staff manual., • Exhibit love for Jesus Christ, the staff, and the clients., • Study the Christian Bible and apply the knowledge received/revealed to job duties and clients' specific issues., • Help the clients make choices that are justifiably in their eternal best interest., • Practice Christ-like stewardship of all resources., • Experience Social Worker, Counselor, or Mental Health professional who knows how to apply theory and put it into practice with an in-depth understanding of the professional field., • Known as competent in the Social Work field., • Organizes and executes multiple projects and is able to re-prioritize when a new issue arises to ensure a timely response., • Makes decisions within broad parameters and understands costs and benefits associated with various options for work processes., • Involves making decisions that affect not only the well being of Light of Life, but also directly the overall reputation of Light of Life, and staff's and client's best interests., • Requires self-awareness and understanding of applying his/her giftedness to serve others and the community., • Bachelor's degree in social work, counseling, psychology, human services, or related field., • Master's degree and/or licensed counselor preferred., • Bible training a plus., • Knowledge from formal and experiential training in drug and alcohol recovery and mental illness., • Two to four years of relevant experience or equivalent combination of education and experience., • HMIS experience and knowledge preferred., • Excellent communication skills (written and verbal)., • Proficient in Microsoft Office Light of Life Ministries Inc offers a comprehensive benefits package including: Medical, Dental, Vision and other valuable benefits, which begin the first of the month following employment. Job Description Clinical Team Leader Summary START is an evidence-based, community crisis prevention and intervention service model for individuals aged 6 and older with intellectual and developmental disabilities and mental health needs. The Clinical Team Leader is a professional position within the Office of Developmental Supports (ODS), Allegheny County Department of Human Services (DHS). This position provides support to the Clinical Coordinator team and overall program system support in a supervisor capacity. The Team Leader requires professional boundaries, and the ability to supervise multiple team members and foster a strengths-based and cohesive goal-oriented team. Essential Duties and Responsibilities • Provide administrative supervision to START Clinical Coordinators and provide daily clinical triage support., • Maintain linkages and relationships with community partners., • In collaboration with program director, work to develop positive relationships with system partners, provider systems, families and/or caregivers., • Serves as a positive representative for the START program with other professionals, families, systems and community., • In collaboration with program and clinical director, coordinate training using the expertise of psychologists, psychiatrists, team members and specialists within the community., • Explore and facilitate opportunities for ongoing in-service and continuing education opportunities for START Clinical Coordinators and team., • Participation in START National Training Institute (SNTI) and START collaborations., • Active participant in referral reviews for enrollment., • Lead/participate in daily Triage meetings with START team and external partners., • Maintain knowledge of various assessments and practices through attending in-service opportunities presented., • Collaborate with other members of the START Leadership team to monitor quality of START service delivery and assure fidelity to the START model., • Ensure all documentation required of Coordinators is completed and entered into SIRS in a timely manner in accordance with START and program guidelines., • Ensure the coordination of support meetings and crisis plans for people served through START., • Share administrator on-call responsibility to the team., • Participate in recurring meetings with START leadership and clinical team., • Development of cross-systems crisis plans, intake/assessments, intervention outcomes and any other applicable documentation of services provided., • Working knowledge of PERMA+, strength spotting and evidence-based assessments provided by the START Team/NCSS., • Excellent written and verbal communication skills when presenting to senior management, stakeholders and system partners., • Knowledge of various computer systems (i.e. MS Word, Outlook, Excel, etc.)., • Knowledge of applicable regulations for the systems involved with enrollees., • Demonstrate creative problem-solving skills that promote maximum effectiveness of both natural and community resources., • Demonstrated excellence in customer service by resolving issues promptly and maintaining high levels of satisfaction., • Excellent organizational, problem solving and analytical skills., • Proven ability to mentor and develop team members, fostering a collaborative and consensus-based environment., • Demonstrate an ability to facilitate, plan, and organize muti-system activities and handle multiple priorities simultaneously.
