Job Description Description POSITION SUMMARY The Senior Safety and Clinical Risk Analyst will help support and lead initiatives related to safety and clinical risk. This position will work to create a culture that supports reporting risks, incidents, and issues concerning the patients, staff safety, and overall quality. This position works with the manager to ensure the organization’s safety and clinical risk programs are implemented to meet HRSA, FTCA,ADHS,OSHA and Joint Commission requirements. OUR CORE VALUES • Inclusion, • Nurture, • Service, • Purposeful, • Integrity, • Resilient ESSENTIAL SKILLS AND EXPERIENCE • High School Diploma or GED from an accredited institution, • Graduation from an accredited medical assistant program, preferred or five (5) years’ experience in a healthcare-related field, • Knowledge of regulatory agency requirements including Joint Commission, OSHA, HRSA, and AZDHS, • Ability to critically analyze safety situations and respond accordingly, • Ability to maintain confidentiality, privacy, and security of protected information, • Ability to draft analytic reports integrating multiple data elements and perspectives, • Excellent working knowledge of Microsoft Office Applications including Outlook, Word, Excel, Teams, and SharePoint, • Knowledge of SBAR, CQI, RCA, ACA, Process Mapping, and PDSA methodology, • Experience with incident reporting systems and investigations, • Experience working with Electronic Healthcare Records (EHR), navigating the system for audits, • Ability to draft analytic reports integrating multiple data elements and perspectives, • Ability to travel to health center locations, • Successful completion of NAHQ Foundational: HQ Principles or completion within six (6) months of hire, • Successful completion of FTCA-approved health care risk management training within twelve (12) months of hire, • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations), • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations, • Comply with the organization’s immunization requirements Risk Assessments • Collaborate with the Safety and Clinical Risk Manager with clinical risk assessments to identify:, • Potential risks and hazards in patient care and safety, • Adverse events, • Patient complaints, • Collaborate with key stakeholders to:, • Investigate clinically related incidents, • Determine root cause and contributing factors, • Assist with ensuring compliance with regulatory requirements related to:, • Patient care and safety, • Accreditation standards, • State and federal regulations, • Stay current with regulatory requirements, • Provide education to staff on:, • Clinical risk management strategies, • Patient safety, • Process improvement initiatives, • Collect and analyze data related to:, • Patient care and safety, • Incident reports, • Patient complaints and grievances, • Quality metrics, • Create analytic reports and present data at weekly, monthly, and quarterly meetings, • Assist with clinical risk initiatives associated with the annual FTCA application, • Lead workgroups to improve FTCA application readiness and associated processes, • Assist with gathering data and completing the Annual Board Report, • Assist with gathering data and supporting documentation for the HRSA OSV, • Develop and maintain robust documentation and reporting of the following:, • Action plans, • CQIs, • RCAs, • ACAs, • FMEA, • Process mapping, • Other safety and clinical risk-oriented activities Provides recommendations and assists with Risk Assessments for the following plans: • Clinical Risk Management Plan, • Emergency Management Plan, • Collaborate in the transition of care coordination for assigned patients who are hospitalized, ensuring continuity of care and adherence to UDS+ grant requirements NONESSENTIAL SKILLS AND EXPERIENCE: • Ability to communicate effectively both verbally and in writing, • Excellent customer service and communication skills, • Fluency in a language in addition to English a plus (Spanish preferred) Additional Duties and Responsibilities • Other duties as requested or assigned Patient-Centered Medical Home (PCMH) All employees are responsible for promoting and participating in interdisciplinary communication and collaboration to enhance the delivery of quality across the health care system.
