SHOPP offers 12 essential programs, including CCNP (Community Care Network Program), 4 Older Adult Centers (Casa Boricua, Leon, Soundview, and Guess), NORC (Naturally Occurring Retirement Communities), Pop Up Caf, Case Management with Friendly Visiting, Elder Justice, Caregiver Support Program, and NY Connects. SHOPPsmissionis to enhance the strength and resilience of individuals, regardless of age or ability, through innovative programs and support services. The Neighborhood SHOPP Violence Intervention and Prevention (VIP) Program focuses on alleviating the effects of domestic violence and prevent victimization to older Bronx and Manhattan community district 12 residents. We believe that older adults should be in a safe environment and be able to recognize and deal appropriately with threatening situations to minimize risk to themselves. The Elder Justice Intake Worker will report directly to the Elder Justice Program Director and/or Assistant Director. The position requires a Bachelors Degree in Social Work or a related field with a minimum of 1-2 years of experience working in a social service/non-profit setting. Knowledge of domestic violence and/or older adult community-based services is preferred. In addition, the candidate must be comfortable with public speaking, reporting to alternate sites, multi-tasking, team building, critical thinking, empathy, time management, cultural competency, and knowledge of using Microsoft office applications while working in a hybrid operation. Job Description: With a Hybrid work schedule, the Elder Justice Intake Worker is specifically responsible for the following: • Determine preliminary eligibility and complete intake along with preliminary assessments on victims seeking information, support, and services;, • Initiate Supportive Counseling at time of Intake for self-referrals;, • Provide information and referral to victims or professionals of other internal/external programs;, • Assist social workers with follow-up calls for telephone reassurance;, • Support training peers/interns;, • Provide Program Director/Assisted Director with completed intake and screening reports;, • Complete and Monitor CD tracker and Referral Link;, • Maintain complete and well-written client case records;, • Complete various worker logs, performance target outcome tracking, and statistical reports in a timely manner amongst other administrative requirements as assigned by a supervisor;, • Involvement in a supervisory relationship with the PD/AD and accept peer-to-peer supervision;, • Coordinate and collaborate with other agencies such as Adult Protective Services, NYPD, the Bronx/Manhattan District Attorney's Office, NYC Aging Elderly Justice Services to ensure that victims are provided with timely and effective intervention;, • Adhere to NYC Aging and other relevant government guidelines, regulations, procedures, and compliance expectations;, • Assist with annual outreach plan to promote Elder Justice program and other SHOPP services in the community, social media, community boards, etc.;, • Conduct Elder Abuse Community Prevention workshops and presentations when needed as a back-up to the team;, • Identify and maintain collaborative relationships in Elder Justice network;, • Coordinate and collaborate with Outreach worker to develop innovative program promotion ideas, etc.;, • Participate in events/conferences during off-hours (after 5 pm) or assist with tabling during community fairs during weekends when needed;, • Attend periodic City Council meetings during off hours as assigned;, • Report to assigned alternative office location as assigned by supervisor;, • Attend periodic training and skill development workshops;, • Participate in SHOPP staff training programs, staff meetings, and other meetings as assigned;, • Perform other tasks as assigned by the Supervisor.
