Job Description Overview & Highlights • Title: Nurse Practitioner, • Location: Mesa, Arizona, • Schedule: Full-time, 4 days per week (Monday-Thursday or Tuesday-Friday, 7:00 AM-5:00 PM), • Compensation: $130,000-$180,000 annually (Base salary starts at $130,000, with average bonus earnings of $40,000-$50,000 per year), • Work Environment: Outpatient wellness clinic, • Coverage Model or Hours: Adult primary care and wellness-focused practice with a 4-day workweek, • Travel: No routine travel required This is an opportunity for a Nurse Practitioner who wants to practice thoughtful, relationship-driven outpatient medicine in a clinic that values both clinical quality and team culture. You will care for an adult patient population in a supportive wellness setting that blends strong primary care fundamentals with an openness to both traditional and holistic approaches. In this role, you will typically see 20-25 patients per day across a balanced mix of physicals, new patient visits, follow-ups, medication checks, pre-op evaluations, and chronic disease management. Common conditions include diabetes, hypertension, and general preventive and wellness concerns. About 20% of visits are completed through telehealth, giving you a varied and modern outpatient workflow. The clinic offers flexibility where it matters. Providers are able to adjust appointment length based on patient needs, allowing for more personalized care and better clinical judgment rather than a rigid, one-size-fits-all model. Documentation is completed in eClinicalWorks, and the day-to-day environment is collaborative, steady, and team-oriented. This is also a strong fit for someone who enjoys growing their skill set. Experience with trigger point injections and joint injections is valued, but genuine interest in learning is welcome. You will be surrounded by a stable, long-tenured team that believes great patient care starts with supporting one another. Why This Role Stands Out • 4-day workweek with full-time hours and better work-life balance, • Stable, team-first clinic culture with strong retention and long-tenured staff, • Adult outpatient setting with a clear, manageable workflow, • Flexibility in visit length to support quality, patient-centered care, • Practice philosophy rooted in transparency, respect, and in-network care, • Nurse Practitioner with strong adult outpatient primary care experience, • Clinician who is caring, diligent, coachable, and grounded, • Team player who works well in a highly collaborative clinic environment, • Provider comfortable managing both in-person and telehealth visits, • Interest in both holistic and traditional approaches to care, • Provide comprehensive outpatient primary care services for adult patients, • See approximately 20-25 patients per day in a wellness clinic setting, • Perform complete physicals, including roughly 4 per day, • Conduct new patient visits, averaging about 2 per day, • Manage follow-up visits for lab review, chronic disease care, and medication checks, • Perform pre-operative physicals, • Evaluate and treat common adult primary care conditions, including diabetes and hypertension, • Manage medication refills and ongoing medication-related follow-up, • Provide telehealth visits, representing approximately 20% of patient encounters, • Perform trigger point injections and joint injections when trained and appropriate, • Accommodate occasional walk-in patients as needed, • Document all patient care accurately and efficiently in eClinicalWorks, • Collaborate closely with clinical and administrative staff to support excellent patient care, • Active Nurse Practitioner license in Arizona, • Active DEA license, • 3+ years of primary care experience, • Strong adult outpatient clinical background, • Ability to manage a patient volume of 20-25 patients per day, • Comfortable providing both in-person and telehealth care, • Strong teamwork and collaboration skills, • Experience with trigger point injections, • Experience with joint injections, • Interest in wellness-focused care models, • Comfort integrating holistic and traditional treatment approaches, • Enjoy adult primary care and want to build real continuity with patients, • Value a supportive team culture and contribute positively to it, • Want a 4-day schedule without sacrificing a full-time role, • Prefer an outpatient setting where care can still feel personal and thoughtful, • Are motivated, respectful, and willing to keep learning, • Prefer highly independent practice with minimal collaboration, • Are uncomfortable with a steady patient volume in a busy outpatient setting, • Do not enjoy chronic disease management and routine primary care visits, • Are not open to coaching, feedback, or team accountability, • Prefer a more transactional or volume-only care model If you are a Nurse Practitioner looking for a stable, collaborative outpatient opportunity where you can practice meaningful primary care, grow your skills, and be part of a team that genuinely supports one another, we would love to hear from you. Please apply with your most updated resume or CV. A cover letter and references are preferred but optional.
