Conducts spot checks of individual medication supply to assure adherence., 9. Conducts venipuncture to obtain blood specimens, obtains urine or saliva samples for toxicology testing., 10. Obtains daily test results from clinical laboratory and assure that all lab reports are filed in the patient record., 11. Alerts prescriber and clinical team immediately upon identification of medication adherence concerns., 12. Alerts prescriber immediately upon receipt in the event of abnormal lab values., 13. Fax, phone, electronically deliver, or by any other practical means, advise pharmacy of medications to be filled., 14. Receives, checks and secures medication deliveries., 15. Responsible for conducting medication inventory at start and end of each shift., 16. Attend regularly scheduled staff meetings and case conferences., 17. Qualifications for Position ▪ New York State Certified Recovery Peer Advocate or Provisional CRPA preferred. ▪ CARC preferred. ▪ Lived experience in recovery from substance use disorder. ▪ 2 years of direct care work experience with individuals at risk for, or with a substance use disorder. Job Description TITLE: Custodian REPORTS: Building Superintendent/Assistant Program Director/Deputy Director of Operations/Facility Manager DEPARTMENT: Operations FLSA CODE: Non-exempt PROGRAM: OASAS Residential/ Supportive Housing MAJOR FUNCTIONS: The Custodial Worker is responsible for all custodial renovations, safety code and emergency situations in the facility. S/he is responsible for daily cleaning and housekeeping activities. SPECIFIC DUTIES & RESPONSIBILITIES: • Conduct regular facility inspections to assess standards and identify areas of concern., • Responsible for emergency and minor repairs to be completed in a timely fashion., • Responsible for responding to consumer work orders in a timely fashion., • Responsible for ensuring bedroom/apartment maintenance, cleanliness and inspection prior to arrival of new consumers., • Ongoing housekeeping and cleanliness of facility of all public areas., • Meet monthly for supervision with assigned supervisor., • Clean floors, windows, fixtures, offices, stairs, restrooms and doors. This cleaning process is normally done with brooms, mops, brushes and cleaning cloths., • Use of various machines such as buffer, vacuum cleaners, carpet machine, etc., • Responsible for the removal of snow in the winter from parking lots, driveways and other areas that might prevent the resident/tenant to enter. The sidewalks are kept clear and ice free with snow melt., • Attend regularly scheduled staff meetings., • Attend all required in-service training seminars., • Other relevant duties as assigned.
