The TransCare Program provides HIV/STI/HCV prevention, education, testing, and linkage-to-care services tailored to the unique needs of Transgender and Gender Non-Conforming/Non-Binary (TGNCNB) communities of color. Position Summary The TransCare Program Coordinator is a key member of IOOV’s LGBTQ+ Health Services team. This role is responsible for implementing evidence-based and homegrown interventions, overseeing HIV/STI/HCV awareness and prevention initiatives, conducting risk assessments, and managing general program operations. The Coordinator will also supervise staff, facilitate outreach, and develop culturally responsive programming to reduce health disparities among TGNCNB communities. Major Responsibilities Program Coordination & Implementation * Coordinate HIV/STI/HCV prevention services for TGNCNB communities, with a focus on TGNC people of color. * Implement, recruit for, and facilitate intervention programs, including Project Phoenix and SexTransations. * Expand testing services to increase the number of TGNC individuals who know their HIV/STI/HCV status. * Provide direct case management, referrals, patient navigation, and linkage to care.
