Job Description Philadelphia FIGHT is excited to welcome our next PrEP Retention Coordinator who will primarily work at our Broad Street Love clinic (BSL), while also supporting PrEP efforts at the John Bell Health Center (JBHC), our young adult and adolescent health center (Y-HEP) and additional clinics throughout FIGHT as needed. The PRC supports the clinical team in PrEP care and plays a key role in the development and expansion of PrEP resources including but not limited to education, recruitment, engagement and retention, patient navigation and adherence counseling at each of the health centers. The Broad Street Love Health Center is an open-access medical clinic that provides urgent care, transitional primary care, and low-barrier treatment for substance use disorders. JBHC and Y-HEP offer holistic and trauma-informed approaches to primary care and aim to be welcoming and affirming for all patients and staff members. The team-centered approach at each of our clinics is essential to addressing the social determinants of health impacting our community and upholds our mission to reduce health disparities for particularly vulnerable populations. About the Role: • Schedule: Full-time, Monday - Friday (9:00 - 5:00PM) may require occasional evenings, weekends, and late nights for outreach events, • Location: Philadelphia FIGHT, 1233 Locust St, Phila, PA 19107, • Compensation & Benefits:, • FSLA status is non-exempt with hourly rate of $24-26 per hour, • 3 weeks of PTO annually + 11 paid holidays, • 403B with up to 4% employer matching that is fully vested starting day 1, • Medical, vision, and dental insurance starting day 1, • FSA for medical, dependent care and transportation expenses, • Discounted degree options with partnered institution Pierce College Requirements: • High School Diploma required, • Bachelor's Degree is preferred, • 1-2 years of experience conducting non-medical assessments and client centered education is preferred, • PA HIV Testing Certification is preferred, • Strong computer literacy with proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint, Teams), • Previous experience using an Electronic Medical Record System (EMR) is preferred, • Knowledge of PrEP and combination HIV prevention and treatment, • Knowledge in science and medicine, • Proven track record of organization, attention to detail and ability to respond promptly to requests, anticipate agency needs, and maintain efficient clinical operations, • Ability to work across teams effectively, • Sensitive to cultural, economic, gender, and sexual orientation differences Responsibilities include: • Conduct non-medical health screenings and risk assessments of individuals who are interested in Pre-Exposure Prophylaxis., • Provide client-centered health education on topics such as HIV & STI prevention, medication adherence, and harm reduction., • Provide ongoing education about injectable PrEP and scheduling of dosing, other alternatives to daily oral PrEP, when and how to stop PrEP if desired, when and how to reinitiate PrEP in the future, when to call a healthcare provider with questions, and when & how to schedule an appointment with our clinics., • Facilitate linkage-to-PrEP care to recruit individuals at increased vulnerability to HIV in Philadelphia., • Field phone calls coming into the PrEP Hotline and provide culturally sensitive, medically accurate information about HIV, Pre-Exposure Prophylaxis and Post Exposure Prophylaxis., • Maintain a database of PrEP patients including tracking of active PrEP patients, patients overdue for injections, labs or provider visits, and patients lost to care and follow up., • Weekly outreach to PrEP patients lost to care and follow up to re-engage in care., • Support clinical staff at BSL, JBHC and YHEP to refer, retain and empower patients in the PrEP program by:, • Confirming PrEP appointments are scheduled for healthcare provider, injection, and lab visits, • Ensuring patients are compliant with guideline-specific bloodwork, • Coordinating medication refills, • Applying for and maintaining eligibility in patient medication assistance programs, • Conducting rapid HIV testing as part of new and ongoing PrEP care, • Facilitating reminder phone calls and text messages to patients with upcoming appointments, • Linking patients to PrEP services who have recently been diagnosed with an STI or have been prescribed Post-Exposure Prophylaxis (PEP), • Scheduling TelePrEP appointments and coordinating at-home and/or outside labs for routine HIV and STI screenings for the patient, • Inputting at-home testing lab results into the EMR for provider review, • Responsible for documenting all patient encounters in the electronic medical records within 48 hours, including but not limited to phone conversations, counseling sessions, HIV rapid test results, medication pick-ups and other patient communication, • Work with the pharmacy department to manage PrEP medication distribution for both oral and injectable medications; assist patients with picking up PrEP prescriptions from contracted pharmacies and deliveries of PrEP medications to the health center, • Work with the managers of the PrEP program to cultivate community partnerships, coordinate outreach, and educational activities around specific health awareness activities, participate in community health fairs as needed, • Support linkage to care efforts for PrEP participants who may be newly diagnosed with HIV., • Refer patients to appropriate programs within Philadelphia FIGHT's Community Health Centers., • Attend scheduled meetings, case conferences, and trainings as required., • Demonstrate respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
