Preferred Qualifications: • Responsible for processing of medication orders for all participants through direct input or processing of medication orders input by staff provider, ensuring that a complete, accurate, and legal prescription is received by the dispensing pharmacy., • Reviews notes from outside providers and identifies and processes medication orders with approval of PACE provider., • Responsible for coordination to maintain participant medication schedules (maintenance of cycle system) through implementation of new medication orders, mid-cycle medication changes, discontinuations, and reconciliations through transition process as prescribed by policy., • Responsible for medication reconciliation through care setting transitions (LTC/Hospital/ALF discharge) for all participants in accordance with organization procedures and facilitates any changes required., • Responsible for communication with and between PACE providers, community-based providers, and the pharmacy in regard to medication orders, clarifications, and pharmacy recommendations., • Responsible for coordinating with local pharmacies to obtain urgent medications that are not available through on-site dispensing. Coordination involves transmission of medication order, demographics, and insurance billing information., • Responsible for coordinating and/or facilitating on-site medication dispenses through the system provided through the pharmacy. Also responsible for periodic inventory and maintenance of on-site medication stock. Job Description Under general direction, the Medical Assistant provides direct patient care to patients of Choice Pain & Rehabilitation Center, LLC. This position is responsible for abiding by HIPPA rules and regulations and assisting patients with medical forms in addition to the below noted essential duties. Candidate must have experience with EMR systems, requesting medical records, utilizing the Pharmacy Database Monitoring Program as directed by providers, recording vital signs and medical history, collecting patient information, facilitating patient flow, and complies with measures to assure that patient/customer satisfaction results are consistently maintained. Medical Assistant Essential Duties and Responsibilities: • Accurately ensuring patient medical records are complete., • Prepares all forms and requisitions as instructed by the provider., • Takes and records patient vitals., • Properly maintains examination rooms and other clinical areas, including supply inventory and maintenance., • Maintains certifications and skills., • Adheres to Infection Control, Safety and OSHA standards as specified by State, and Company standards., • Maintains knowledge of, and complies with established State, Company and any other applicable policies and procedures., • Instructs patients in simple procedures/tests and dispenses appropriate educational materials., • Draws up injections to be administered., • Verifies that radiological and laboratory records are received and in the patient chart. Job Description Overview of Position - The Community Living Delegating Nurse (RN CM/DN) for Ardmore Enterprises is responsible for assessing and overseeing the nursing care needs of people supported by the organization. The RN CM/DN will visit the community living group homes, as well as people supported in Supported Living and Personal Supports (where applicable) routinely, based on assessed need. The RN CM/DN is responsible for completing Initial Nursing Assessments, routine 45-day assessments, or more frequently if necessary and provide the organization with a written review. The RN CM/DN will develop comprehensive nursing care plans annually, or more frequently based on assessed need, train staff on nursing care plans, evaluate staff proficiency with medication administration and appropriate care of the person supported, review medical records, and serve as a support to the Services departments. The RN CM/DN would be responsible for maintaining status as a DDA RN CM/DN by attending 2 annual DDA Nurse meetings and providing annual clinical reviews of the (HRST) Health Risk Screening Tool. The Delegating Nurse will report to the Nurse Manager and/or Senior Director of Services and collaborate with her/him to coordinate care of people supported and teaching the MTTP (Medication Technician Training Program). Case Management/Delegating Nurse Responsibilities: • Provide Nursing Case Management and delegation functions to include, on-site visits based on assessed need, but at a minimum of every 45 days. Visits should include an individual assessment of health status, evaluation of medication administration delivery system, including a review of the Medication Administration Record(s) (MAR’s), Medications and Physician Medication Order Forms (PMOF’s) for those clients needing nursing delegated duties. (This time frame is based on original MBON and DDA guidelines and will be adjusted as directed by the MBON and DDA in State of Emergency situations.), • Delegate medication administration and approved nursing tasks to Certified Medication Technicians (CMT) in the community living setting., • Provide established nursing on-call coverage for medical/nursing related matters as scheduled., • Provide evaluation of competency of Medication Technicians., • Provide instruction in the Medication Technician Training Program as needed by the organization., • New Admissions: Assess the individual, review medical history, current medications, setting for delegation and make recommendations to designated staff. Documents all patient information on medical charts and follows physician’s written orders. Maintains patient confidentiality. Maintains accurate records and documentation of patient treatments, immunizations and laboratory results according to standard medical recordkeeping. Performs administrative assignments such as filing, answering phones, typing, and completing progress notes in the computer system. Job Description Overview of Position: The Nurse Manager (NM) for Ardmore Enterprises is a registered nurse with demonstrated expertise in case management, leadership, and management skills, demonstrating a high degree of professionalism, directs the planning, implementation and evaluation of care and management of I/DD populations. The NM works collaboratively with all departments to ensure that nursing services are delivered at the appropriate level in keeping with DDA and MBON regulations. The NM is responsible for creating an environment conducive to achieving quality outcomes, effective use of resources and retention of quality staff. He (she) is responsible for the management and development of his (her) resources, human and fiscal if needed, to include implementation of quality control, quality improvement, satisfaction of supported individuals, families, and staff as well as review and revision of policies and procedures, as needed. The NM collaborates with other department leaders on issues related to the effective management of health and safety and interacts regularly with the appropriate resources to ensure the effectiveness of care. He/she will also act as liaison between the organization and DDA, associated hospitals, long term care, or rehabilitation services as needed. The NM will assume a small caseload within the Meaningful Day services department. The NM is a resource within the organization regarding nursing services, and related issues within the community.
