As part of New York State's 1115 Medicaid Waiver, NSIPA plays a key role in identifying, screening, and addressing health-related social needs by connecting Medicaid members to community-based resources and care supports. These services are designed to strengthen provider efficiency, address health related social care needs, and advance value-based care and population health opportunities. Salary: $150000 Position Overview: We are seeking a dedicated and patient-centered Nurse Practitioner (NP) to join our Avenue U provider office. As part of an Independent Practice Association (IPA), this role combines traditional clinical care with responsibilities that support quality performance, population health, and value-based care initiatives. The NP will work closely with clinical and administrative staff to deliver exceptional care, improve patient outcomes, and meet IPA quality benchmarks. Roles & Responsibilities: Perform comprehensive patient assessments, including medical history, physical exams, and diagnostic evaluations. · Diagnose and manage acute and chronic conditions within scope of practice · Develop and modify treatment plans based on evidence-based guidelines · Prescribe medications and therapeutic interventions in accordance with state and federal regulations · Provide ongoing patient education regarding disease prevention, medication adherence, and lifestyle changes. · Conduct wellness visits, follow-up visits, and urgent care sick visits as needed · Support closing care gaps (e.g., annual wellness, screenings, chronic disease follow-up). · Work closely with physicians, nurses, medical assistants, front desk staff, and care management teams · Assist in developing and maintaining clinic protocols, workflows, and patient care standards. · Collaborate with IPA care managers, quality teams, and payers to coordinate patient outreach and care plans · Review patient panels to identify high-risk or non-compliant patients; assist with outreach and follow-up planning · Document all encounters in the EMR accurately and timely, including appropriate coding and visit justification · Adhere to HIPAA, OSHA, and regulatory guidelines · Follow clinic protocols, IPA standards, and evidence-based practice guidelines. Skills & Competencies: Strong clinical judgment and diagnostic reasoning · Ability to work autonomously and collaboratively · Culturally competent and sensitive to diverse backgrounds · Excellent communication and interpersonal skills · Ability to prioritize and manage time effectively in a busy practice setting · EMR proficiency (eClinicalWorks, Epic, or similar systems) Required Qualifications: 1–2 years NP experience in outpatient or primary care setting (new grads considered based on fit) · Current & unrestricted New York State NP license · Master's or Doctorate degree in Nursing (NP program accredited). · EMR proficiency (eClinicalWorks, Epic, or similar systems). · Experience in a value-based care environment, IPA, ACO, or managed care setting Why Join NSIPA?
