Job Description Family Medicine Physician Department: Clinical Operations Reports To: Chief Medical Officer FLSA Status: Exempt Position Summary: The Family Medicine Physician provides comprehensive primary care services to a defined panel of patients within Nuvia Medical’s value-based care delivery model. The physician is responsible for achieving optimal health outcomes, improving quality performance, and reducing unnecessary utilization through proactive management of patient panels. Care is delivered in collaboration with care coordination staff and the Quality Assurance team, as well as other Nuvia Medical stakeholders, to close gaps in care, meet HEDIS and Continuity-of-Care (COC) measures, and reduce avoidable ER visits and hospitalizations. This role is critical to Nuvia’s mission of providing compassionate, accessible, and high-quality healthcare to underserved populations. The majority of patients are Medicaid members, with a smaller portion covered under Medicare and fee-for-service plans. The physician must demonstrate strong clinical judgment, patient engagement, and accountability for outcomes in a population health environment. Essential Duties and Responsibilities: • Deliver full-spectrum outpatient primary care to patients of all ages, focusing on preventive health, chronic disease management, and early intervention., • Manage a defined patient panel under a per-member-per-month (PMPM) model, maintaining responsibility for quality, utilization, and patient satisfaction outcomes., • Identify, monitor, and close gaps in care by collaborating with care coordination staff and the Quality Assurance team., • Ensure compliance with HEDIS quality metrics and other payer-defined performance standards that impact shared savings and incentive programs., • Promote appropriate utilization of healthcare resources to minimize avoidable ER visits, urgent care use, and hospital admissions or readmissions., • Develop individualized care plans for high-risk or medically complex patients to support improved outcomes and reduced total cost of care., • Participate in population health initiatives designed to improve chronic disease control, preventive screening rates, and medication adherence., • Work closely with care coordination staff and the Quality Assurance team to analyze clinical data, review performance dashboards, and implement targeted interventions to improve results., • Educate and empower patients and families to actively manage their health through counseling, goal-setting, and consistent follow-up., • Document all encounters accurately and timely within the electronic health record (EHR) in accordance with organizational and regulatory standards., • Maintain up-to-date knowledge of current clinical guidelines, value-based care best practices, and quality improvement methodologies., • Contribute to interdisciplinary meetings, peer reviews, and continuous improvement initiatives., • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution., • Board Certification or Board Eligibility in Family Medicine., • Current, unrestricted license to practice medicine in the State of Florida., • Active DEA registration and prescriptive authority., • Minimum of two (2) years of experience in an outpatient primary care or value-based care setting preferred., • Proven ability to manage patient panels, track performance metrics, and improve outcomes in a population health model., • Knowledge of HEDIS measures, managed-care quality programs, and risk-based payment structures., • Experience serving Medicaid and underserved populations strongly preferred., • Excellent interpersonal and communication skills with a team-based, patient-centered approach to care., • Population Health Management: Demonstrates accountability for the health outcomes of an assigned patient panel., • Quality & Compliance: Understands and applies quality metrics such as HEDIS and ensures all applicable measures are met or exceeded., • Team Collaboration: Works effectively with care coordination staff and the Quality Assurance team to close care gaps and meet organizational goals., • Resource Stewardship: Uses sound clinical judgment to reduce unnecessary utilization while maintaining high standards of care., • Integrity – Acts ethically, honestly, and consistently in all interactions with patients and colleagues., • Compassion – Demonstrates empathy and kindness while respecting the dignity and individuality of each person., • Accountability – Takes ownership of clinical decisions, patient outcomes, and performance metrics., • Respect – Values the contributions, perspectives, and diversity of all team members and patients., • Excellence – Strives for superior clinical and operational performance in alignment with Nuvia’s value-based mission.
