If you’re ready to bring your passion and expertise to a supportive and rewarding environment, we’d love to have you on board!What does success look like in this role? • Collaborative Spirit: Actively participate in the Interdisciplinary Team (IDT) by sharing valuable insights from assessments, contributing care plan recommendations, and ensuring seamless care coordination., • Comprehensive Assessment Skills: Conduct face-to-face nursing assessments that encompass physical, psychosocial, and behavioral health, meeting participants where they are—whether at home, in our day center, virtually, or in a long-term care facility., • Participant-Centered Care: Partner with medical providers to deliver personalized care for a dedicated panel of participants, ensuring their unique needs and care plans are prioritized., • Clinical Excellence: Deliver nursing interventions with precision and care, accurately responding to physician orders, administering medications, and executing therapeutic interventions., • Decisive Action: Use your triaging expertise in outpatient settings, initiating therapies within your scope of practice, and working with medical providers to escalate care as needed., • Proactive Case Management: Navigate transitions of care with ease by maintaining strong communication with hospital and long-term care facility case managers to ensure continuity and collaboration., • Effective Educator: Empower participants, their families, and team members by educating them on how to implement and sustain personalized care plans with confidence., • Detail-Oriented Professionalism: Maintain accurate, timely documentation of all participant interactions and clinical interventions in the electronic medical record (EMR), adhering to regulatory guidelines from DHCS and CMS., • Crisis Response Leader: Respond swiftly to medical emergencies, coordinating with external providers and family members to provide effective care in urgent situations., • Team Player: Step in to support the team as needed, contributing your skills and expertise in ways that make a meaningful difference.What does an ideal profile look like, • Mission-Driven Passion: A deep commitment to serving high-risk seniors and frail older adults, helping them live with dignity and independence., • Proven Expertise: At least 2 years of experience caring for medically complex or older adult populations preferred, with strong clinical acumen in managing complex interventions., • Exceptional Communication: Active listening and conflict management skills to foster trust and collaboration with participants, families, and team members., • Flexibility and Adaptability: Thrives in multiple clinical settings, including homes, day centers, and long-term care facilities., • Hands-On Clinical Skills: Demonstrated experience in wound care, IV therapy, phlebotomy, and colostomy/ileostomy care, among other clinical procedures., • Licensure and Education: Active, unrestricted RN license in California. Graduate of an accredited School of Nursing; BSN strongly preferred. Proof of current CPR/BLS certification or ability to obtain within 60 days of hire., • Versatile Healthcare Experience: Bring valuable experience from diverse healthcare settings such as PACE, Hospice, Adult Day Health Care (ADHC), Inpatient, and Outpatient environments. This background equips you with the adaptability and knowledge to thrive in our dynamic and multifaceted care model., • Specialized Training: Alzheimer’s certification is a plus but not required., • Bilingual Abilities: Fluency in English and Spanish is preferred, enabling meaningful communication with diverse participants., • Safety and Compliance: Proof of a valid CA driver’s license, personal transportation, a good driving record, and auto insurance as required by state law.The Fine Print, • Your home base: Los Angeles, CA (90057), • Your rhythm: Monday–Friday, 8:00 a.m.–5:00 p.m., • Where you’ll work: 100% onsite at the clinicWhere You'll Work:, • A modern clinic setting: exam rooms, treatment areas, nurses’ station, and lab draw space—with a steady, team-based pace., • Regular use of clinical tools (BP cuffs, thermometers, pulse oximeters, glucometers, EKG) and everyday office tech (EMR, multi-line phone, printer)., • You’ll move between rooms—and sometimes floors—throughout the day; stairs may be used when elevators are busy., • Safe care is our priority: appropriate PPE provided (gloves, masks, eye protection; N95 fit testing may be required)., • Expect normal clinic sounds (phones, conversations, equipment alarms) and occasional exposure to cleaning agents and biohazard materials—with proper protocols in place.What Is the Physical Demand?, • Comfort working independently and making sound clinical decisions in a dynamic environment., • Lots of conversation: you’ll speak, teach, and coordinate care for much of the shift., • A mix of movement: long periods of sitting (documentation, calls) and standing/walking (rooming, triage, procedures)., • Fine motor skills for injections, venipuncture, wound care, point-of-care testing, and EKG lead placement., • Ability to navigate the EMR and other systems independently for documentation, orders, and care coordination., • Visual/hearing acuity for reading labels and chart details, auscultation, and responding to equipment alarms., • Strength to lift/carry up to 25 lbs. (supplies/equipment); occasional assist with adult patient repositioning or transfers (with teammates/devices) and pushing/pulling wheelchairs or equipment., • Flexibility to bend, kneel, reach, and stretch as procedures and safe patient positioning require., • Ability to use stairs as needed and, on occasion, travel between nearby clinic sites for meetings or training.
