Are you an experienced hospice nurse looking for one or more of the following? • PT flexible hours., • Hospice Nurse who wants to be part of a team, help create and form the best hospice in this area! Position Involves: • Drive to homes, assess and cares for hospice patients., • Provide instruction and guidance for caregivers., • Work closely with the hospice team to provide outstanding hospice support for our patients and their loved ones., • Provide case notes on simplified state of the art technology. Responsibilities: The HospiceAdmission Registered Nurseis the first member of our hospice team with whom patients come into contact and plays a pivotal role in the educational process for patients, families, and caregivers. You are the very first face they see as they embark on their journey. As a Hospice Field Admission Nurse (RN), you will admit eligible patients onto the Hospice program following the appropriate standards of practice established by Medicare Guidelines. In this field position you will perform initial physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care, goals, and make recommendations based on each patient's needs. Assessments may take place in private homes, hospitals or nursing facilities. Qualifications: • Current California Registered Nurse (RN) licensure (in good standing) with one year of experience, • Provides compassionate education concerning hospice care philosophy, as well as the nature of the care they can expect to receive in regard to their specific terminal illness, • Guides patients, families, caregivers, community partners through the hospice assessment and admissions process., • Work closely with Clinical Leadership, Physician, Intake Coordinator, and Liaison to understand the patients needs and determine patient's eligibility for hospice care., • Utilizes strong clinical skills to evaluate hospice eligibility per Medicare guidelines., • Ability to learn an electronic medical records system (HOSPICEMD) Job Type: Part-time Weekly schedule: • Monday to Friday, • Weekends as needed Experience: • Hospice: 1 year (Required) License/Certification: • Registered Nurse (Required) Work Location: On the road remote work

Responsibilities: • Evaluate patient/family needs in the preparation and execution of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors the patient's reactions, • Complete patient/family initial and ongoing assessments to determine hospice needs, re-assess and update the plan of care, • Provide direct skilled bedside nursing care, • Evaluate/provide/educate/maintain a safe environment for the patient, • Effectively utilize resources to assist in meeting patient's needs, • Contribute to providing hospice continuous care according to the plan of care and prescribed doctor's orders, • Experience in hospice care visits is required, • Compassionate and loves to listen to / educate patients, • Proficiency in computer/ smartphone for electronic notes use, • Knowledge of HospiceMD software is required, • Bilingual is preferred, • Valid State license, • Understanding of hospice philosophy and needs of the terminally/seriously ill, • Good communication and organizational skills.
