IntellaTriage Nurses enjoy the benefits of working from home, providing care, without the stress and expense of commuting each day! **MUST have or be willing to obtain a Compact RN license **MUST live in/work from a Compact US state **Experience with end-of-life care is required **Must have high speed internet **Must be tech savvy, enjoy a fast-paced environment, and have keyboard competence **Part-time nurses only work 4-6 days out of a 14-day pay period Part- time schedule: • Work a minimum 3 evening shifts per pay period 3:30p-11p CST (shift times are set/ week day flexes) Our Hospice Triage Nurses: • Have experience in hospice, palliative, end-of-life care and in fast-paced environments: ED, critical care, surgical services, etc., • Part-time nurses work a minimum of 1 week day shift & every other weekend both Saturday & Sunday, • You can pick up additional shifts, if available, for the clients’ you are trained to support., • We will provide you with a laptop and headset., • You’re required to use your own high-speed internet, • You’ll need a quiet space, away from noise and distraction, while you work (privacy/HIPAA compliant space is required)., • You’ll access EMRs for charting and utilize our internal applications to perform job functions, • You MUST be able to follow instructions, read directions, and be confident using technology, • A minimum of 30 minutes paid shift prep is required prior to taking calls Then… put those days on the floor, that commute, and those hours on the home-visit road behind you! Read more about this award and its accompanying benefit here: Requirements • MUST have or be willing to obtain a Compact RN license (states with pending or future implementation dates are not considered current compact states until the implementation date), • You must remain in good standing and ensure your home state license remains active., • IntellaTriage will cover the cost of non-compact state licensure if necessary for client support., • Hospice, palliative, end-of-life care is strongly preferred, • Experience in a fast-paced environment: ED, surgical services, or critical care, etc., • Must be comfortable accessing multiple technology applications to document during calls, • Ability and comfort with typing in a fast-paced environment, • Fluency in English is required, additional languages are a bonus, • Must physically reside in the U.S. and be legally eligible to work for any employer, • Must be able to complete three weeks of remote paid training that is conducted during days and evenings, • Must be available to work every other Saturday & Sunday, • Must be available to work some Holidays as required, • Must be able to handle stress and multitask when receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends), • Must be able to communicate empathically with patients while adhering to protocols, • Must maintain CEU’s as designated by the states in which you are answering calls, • Must attend any in-services, and additional training on an as needed basis All Remote Hospice Triage RNs, once trained to their originally assigned team are paid $23 per hour. There are multiple opportunities to increase the hourly rate. All nurses are eligible for a $1 shift differential for overnights and a $1 shift differential for weekends (Saturday & Sunday). All part-time and full-time nurses accumulate PTO, based on the number of hours worked (per year). All part-time and full-time nurses are eligible to participate in our 401(k) plan. Full-time nurses may also participate in medical, dental, vision, and/or supplemental insurances.

Job Description We're seeking a passionate and highly motivated Nurse Care Manager to join our dynamic Care and Case Management team. In this telephonic role, you'll play a crucial part in holistically guiding our members through their healthcare journey, ensuring their needs are met with industry-leading interventions. This position places a special emphasis on proactive engagement with high-risk and rising-risk members, particularly around discharge planning and care transitions. You will partner with a multidisciplinary clinical team that includes a variety of healthcare professionals, care coordinators, and records specialists, to deliver integrated remote care in an innovative way. The ideal candidate will enjoy spending time on the phone, actively listening to members’ needs, answering questions, and serving as a dedicated advocate. You should excel at creating cohesive care plans and possess the clinical acumen to guide members through complex situations, leveraging available benefits and resources. Schedule: • CA Based/Licensed: Monday - Friday, 9:00 AM - 6:00 PM PST, • Non CA Based/Licensed: Monday-Friday, 9:00 AM - 6:00 PM local time zone Responsibilities: • Engage in high-value Care Management outreach calls for Included Health members with the primary goal of clinically engaging high-risk and rising-risk members, fostering strong relationships and promoting proactive health management., • Make proactive calls to members once their hospital discharge is confirmed, conducting thorough intake assessments if the member is reached., • Collaborate with hospital-based case managers to understand & support members' specific discharge needs and actively encourage members to engage with our Care Management program., • Perform initial assessments encompassing activities of daily living, cognitive functions, social determinants of health (SDOH), health beliefs and behaviors, and life planning activities to develop truly holistic care strategies., • Deliver coordinated, patient-centered virtual Care Management by telephone and/or video that consistently improves members’ health outcomes., • Generate impactful care plans collaboratively with members and our multidisciplinary care team, empowering members to achieve their desired health goals., • Bachelor of Science in Nursing (BSN), • Compact Nursing License with additional Active California Registered Nurse (RN) and Illinois License in good standing., • 5+ years of experience in clinical nursing, • 2+ years of experience working in care management, case management, and/or disease management, preferably within a health plan, health navigator, or third-party administrator (TPA) environment., • Remote care or case management experience, • Comfortable discussing a wide variety of medical conditions and experienced working with populations across all age ranges. $33hr to $40hr Exact compensation may vary based on several factors, including skills, experience, and education. Job Description The Care Advocate Nurse oversees initiatives surrounding assessing the severity of the injured workers’ reported injury(ies), reviews medical data in CareMC, validates and secures medical information, assesses and evaluates prescribed treatment of work related injury. The Care Advocate Nurse functions as a nurse consultant, supporting the goals of the Claim Management department and of CorVel. This is a remote opportunity. ESSENTIAL FUNCTIONS & RESPONSIBILITIES: • Initiates and receives telephonic contact with reported injured worker, employer and medical providers to obtain treatment plan, secures medical documentation, and assesses if injured worker has returned to work, • Responsible for detailed documentation within the claim system focusing on medical condition, treatment plan and return to work status, • Directs callers to appropriate medical and/or claim resources, • Identifies and communicates urgent situations related to treatment or patient condition directly to the claim team, • Functions as nurse consultant to the claim team, • Assesses appropriate medical follow up and makes necessary recommendations to the claim team, working closely with team to assist in identifying potential red-flags from the injury that may require further follow-up or additional services, • Adheres to the Care Advocate Nurse production standards and Account special handling guidelines, • Additional projects and duties as assigned KNOWLEDGE & SKILLS: • Fundamental concepts, practices and procedures of early intervention in the field of Workers’ Compensation (WC), • Ability to write and speak clearly, easily communicating complex ideas, • Ability to skillfully manage multiple, complex projects and competing priorities concurrently while working under pressure to meet production standards deadlines and maintaining strong customer service orientation, • Computer proficiency and technical aptitude with the ability to utilize MS Office including Outlook, Word, Excel spreadsheets, • Strong interpersonal, time management and written communication skills, • Great attention to detail, and results focused, • Ability to work independently and as part of a team EDUCATION/EXPERIENCE: • Minimum of 2 years clinical nursing experience, • Graduate of an approved accredited school of nursing, • Current unencumbered RN Licensure in state of residency and practicing state(s) must be maintained throughout employment with CorVel PAY RANGE: CorVel uses a market based approach to pay and our salary ranges may vary depending on your location. For leveled roles (I, II, III, Senior, Lead, etc.) new hires may be slotted into a different level, either up or down, based on assessment during interview process taking into consideration experience, qualifications, and overall fit for the role.
