Job Description Position Summary: The Programs Case Navigator will assist and support the efforts of the Coordinated Entry Systems (CES) team and the Programs case managers. This is a hybrid position with the responsibility of CES referrals, case management, and housing navigation. It will interface with Programs clients, property owners, and staff from partner agencies. The Programs Case Navigator will be responsible for identifying and assisting with SSI/SSDI if applicable, following through with housing plans for tenants/clients in any CHI program/project, and work collaboratively with other community organizations to achieve client success. The Programs Case Navigator’s position will be based in Jacksonville and serve Duval, Baker, Clay, and Nassau counties which make up the FL-510 Continuum of Care (CoC). This position will work closely with member agencies regarding housing and supportive services within the four-county area. The Programs Case Navigator is expected to assist clients in a kind, caring, and trauma-informed way to assess, plan, implement, monitor, and evaluate actions required to meet the client’s housing and service needs. The position will include both office and field work. The Programs Case Navigator is expected to function independently, exercise initiative in day-to-day activities, based on expertise accumulated through education, training, experience, and reference to relevant professional literature. This position will coordinate the referral component(s) of the CES, provide case management assistance, and specialize in identifying available housing, matching eligible households to available housing opportunities, and ensure the HMIS database accurately reflects this information. All CES activities should connect eligible households to housing solutions as quickly as possible to make homelessness rare, brief, and non-reoccurring. Position Responsibilities: · Manage the referral component of CES to ensure it is functioning strategically to end homelessness in Duval, Clay, and Nassau counties. This includes but it not limited to cultivating and maintaining partnerships with housing and service providers · Develop, recommend, and implement modifications or revisions to the referral system as needed · Facilitate regular meetings of housing and service providers to discuss housing opportunities and match eligible households to those opportunities · Work alongside the CES Director to review CES outcomes, especially referral-specific outcomes and timelines to ensure compliance with CES policies and procedures · Facilitate ongoing training and support to partners in alignment with referral expectations outlined in the CES policies and procedures · Ensure adherence to best practices and company policies and procedures · Maintain confidentiality of all staff, agency, and client information · Maintain a safe and healthy working environment by adhering to organizational standards and legal regulations · Other duties as assigned Position Requirements: · Educational background can be diverse; however, a degree in a related field or a minimum of three (3) years of relevant work experience preferred · Experience working or volunteering in nonprofit or government settings addressing housing or other social services preferred · Lived experience with homelessness preferred · Veterans and/or experience working with Veterans preferred • Must be eligible to work within the U.S. and provide supporting documentation, • Must pass a Level II background check, • Must pass a federal-level drug screen; possession of a medical marijuana card is currently not acceptable under federal law, • Must have a clean 3-year driving history, • Must provide proof of auto insurance, have a valid driver’s license, and a registered vehicle without known issues or faults to complete essential job functions Knowledge, Skills, and Abilities Required: · Dependability: Demonstrate initiative in setting priorities, regularly completing work on schedule and fulfilling commitments · Relationship Management: Develop and maintain harmonious relationships to ensure positive and productive program outcomes for clients and partner agencies · Innovation: Explore and suggest new approaches and methods to improve the referral component of CES · Communication: Ability to communicate clearly and effectively via oral or written means. Ability to present a friendly and positive demeanor to clients served, service providers, staff, and the general public. Ability to prepare and present on CES and Programs to external audiences · Composure: Ability to remain calm and maintain self-control in difficult circumstances and emergencies. Ability to respond professionally in all situations, including unsheltered conditions. Ability to assist staff with professionalism in complex circumstances with clients and partner agencies. · Decision-Making: Ability to set work priorities and evaluate and create solutions to work-related problems Physical Demands: · Periods of walking, standing, or sitting in an office or field environment for service provision · Limited physical effort required; however, the employee must occasionally lift and/or move up to 15 pounds · Ability to operate a motor vehicle Non-traditional candidates are welcome to apply.
